Description

Summary

Under the supervision of a physician, the Nurse Practitioner or Physician Assistant is responsible for delivering care to patients in an out-patient setting; serving as a liaison with the medical community and making referrals to physicians while being friendly, courteous, and empathic to the patients and all parties involved with and in their care.

Essential Duties and Responsibilities

  • Performs assessment, by obtaining patient medical and social histories, and by performing diagnostic testing and taking vital signs.
  • Evaluates signs and symptoms of patient and orders diagnostic tests.
  • Interprets and evaluates diagnostic tests and laboratory results.
  • Monitors and reviews patient care and treatment plans, which may include nutrition and exercise plans.
  • Provides education and counseling of patients and family in preventive care, medical conditions, and use of prescribed treatments and drugs.
  • Attends staff meetings and serves on committees as needed.
  • Participates in quality assurance activities.
  • Refers patients to physicians or other health care providers and facilities as needed and monitors outcome of the referral.
  • Provides on call coverage for evenings and weekends as needed.
  • Participates in professional development activities and maintains professional affiliations.
  • Maintains patient confidentiality.

  • Compiles information and prepares reports.
  • Maintains records.
  • Participates in educational activities.
  • Follows organizational policies and procedures, including State of Franklin Healthcare Associates Compliance Manual, Code of Ethics and Business Standards, HR Policies and Procedures Manual, Team Member Handbook, OSHA Manual and any other policies or procedures for the job function, department, location or clinic.

Education

Nurse Practitioner - Bachelor degree in nursing and successful completion of a nurse practitioner program at the master level

Physician Assistant Graduate of accredited PA program

Experience

Three or more years of professional experience as a nurse practitioner or physician assistant, ideally in the specific area

Certificates, Licenses, Registrations

Currently licensed in the State of Tennessee as a RN and ANCC certified as a nurse practitioner in a specialty area.

Working Conditions

Work is performed in a medical office environment involving contact with patients, physicians, staff, insurance carriers and other business partners. Occasional local travel between work sites along with travel to educational programs outside of local region and additional hours including evening or weekend work, may be necessary. Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination. Requires standing and walking for extensive periods of time. Occasionally lifts and carries items weighing up to 25 pounds. Requires corrected vision and hearing to normal range. Potential exposure to communicable diseases or bodily fluids. Rapid mobility may also be required on occasions.
