Position Summary: As a Social Worker, you will be an integral part of our interdisciplinary team, delivering medical social services under physician guidance. Your role will focus on assessing patient and family needs and participating actively in their care coordination.

Key Responsibilities:

• Conduct comprehensive assessments within 5 days of patient admission.
• Collaborate with the interdisciplinary team to ensure holistic care.
• Provide assistance with advance directives, funeral planning, and family counseling.
• Facilitate changes in care levels as needed by patients or families.
• Maintain effective communication with patients, families, and team members.

We Value:

• A Master’s of Social Work (MSW) degree from an accredited institution.
• State licensure and a valid driver’s license.
• At least one year of experience in palliative care or hospice settings.
• Proficiency in community resource navigation and computer skills.
• Excellent interpersonal abilities and a customer-focused approach.
• Reliable transportation with valid auto liability insurance.
