Assist the client in learning self-care and health maintenance as appropriate.

Prepare clinical notes for each visit and case conference reports or communication notes as needed.

Conducted the initial and ongoing comprehensive assessment of the client’s needs, including the OASIS upon admission and all other OASIS time points.

Develops written aide care plans and other staff assignments.

Make visits that require a specialized nursing skill.

Complete OASIS documentation, nurses notes, communication records, and case conference reports.

Provide discharge planning as well as a discharge visit with OASIS and written discharge instructions.
