Facility Description

Farmington HealthServices (AKA Trinity Campus) is a five star rated senior living community.

  • Trinity Terraceprovides independent and assisted living services.

Position Description

• Adhere to, supportand promote St. Francis Health Services’ Mission and Core Values, philosophy,goals and objectives while conducting company business.

• Work with theDirector of Home Care to ensure that Home Care is in full compliance withfederal, state, local law and company policies and procedures.

• Recommenddevelopment of and changes to policies and procedures.

• Monitor standards ofcare, service and productivity, ensuring a high quality of life for tenants.

• Communicate, asappropriate, with staff, physicians, referral sources and tenants.

• Establishinformational systems which aid employees to fully understand Home Care’smission, philosophy and operations.

• Provide tours toprospective tenants and actively market the care center.

• Provide in-servicesfor all staff and ensure all staff meets in-service requirements.

• Represent Home Carein dealings with outside agencies, including governmental agencies and thirdparty payers and at various meetings as necessary.

• Verify and approveHome Care employees’ electronic timecards each pay period according to companypolicy and procedures.

• Disciplinespersonnel as requested or as may become necessary and ensures any disciplinaryaction is administered fairly.

• Ensure that all careis provided in privacy, as appropriate, and to knock before entering a tenant’sresidence.

• Ensure servicesatisfaction through tenant visits and other measures as appropriate.

• Responsible forinvestigating and resolving all tenant complaints.

• Complete move-inpaperwork for tenants (i.e. lease agreement, apartment inspection, tenanthandbook and consumer guide).

• Utilize listeningskills that indicate understanding and promote accurate interpretation ofothers' concerns, motives and feelings.

• Work towardresolution of interpersonal conflicts as they arise.

Qualifications

  • Experience in Healthcare or Housing/rental management preferred.
  • Direct care experience a plus

Job Summary:

The primary responsibility of yourjob position is in the management and oversight of the day-to-day functions ofthe Assisted Living in accordance with current federal, state, and localstandards, guidelines, and regulations that govern Assisted Living to ensurethat the highest degree of quality of care is maintained at all times.

Job Requirements:

  • Must possess a current, unencumbered, active license in Minnesota as a licensed Assisted Living Director or commit to obtaining a license within one year of employment.
  • Must be knowledgeable of computer systems, system applications, and other office equipment.
  • Two years’ experience in home health and/or housing.
  • At a minimum must have one year of management experience.
