The Opportunity

St. Agnes Hospital is actively recruiting a full time (1.0FTE) Nurse Practitioner to join our experienced and cohesive behavioral health team. We are seeking an experienced clinician to provide mental healthcare services to children and families with psychiatric disorders.

SSM Health St. Agnes Hospital - Fond du Lac 

Founded by Mother Agnes Hazotte and the Congregation of Sisters of St. Agnes, SSM Health St. Agnes Hospital treated its first patient in 1896. The 161-bed hospital has grown into both an inpatient and outpatient facility providing innovative community and regional health care services to the greater Fond du Lac area. Requirements:

Master's degree from nationally accredited Nurse Practitioner program.

Job Summary:

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

• Initiates and provides medical interventions in response to acute changes in patient conditions.
• Provides educational services to nursing and ancillary departments and serves as an educational consultant on clinical practice issues.
• Applies evidence based practices to achieve multidisciplinary collaboration to facilitate continuity of care.
• Contributes to the satisfaction of the medical staff by collaborating with physicians to deliver best care.
• Recognizes and utilizes opportunities to act as a change agent by introducing nursing practice innovations.
• Evaluates nursing practices associated with specialty area, defines needs/problems with patient care and makes constructive suggestions that creates a positive patient outcome.
• Works in a constant state of alertness and safe manner.
• Performs other duties as assigned.

EDUCATION

• Completion of a formal post-basic education program from or formally affiliated with an accredited college, university or hospital (Before July 1, 1998) or a graduate degree from an accredited college or university (After July 1, 1998) with a concentration in the appropriate advanced practice nursing clinical specialty area.
• Board certification by a national recognized certifying board in the appropriate population focus
• Current or compact RN license in appropriate state
• Appropriate APRN license or document of recognition in appropriate state

PHYSICAL REQUIREMENTS

• Constant use of speech to share information through oral communication.
• Constant standing and walking.
• Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
• Frequent sitting, reaching and keyboard use/data entry.
• Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
• Frequent use of smell to detect/recognize odors.
• Frequent use of hearing to receive oral communication, distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.
• Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
• Occasional lifting/moving of patients.
• Occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive foot/leg and hand/arm movements.
• Occasional driving.
• Rare crawling and running.
