Social Services Assistant assists the facility social worker in providing services to residents in the nursing facility and aid in the access of services to meet residents’ needs.
The Social Services Assistant assists residents and families in identifying and obtaining available benefits and social and community services. They also assist the facility’s social worker with completing admission documentation and facilitating the resident discharge process.
Qualified candidates must have human services related work experience, and have excellent verbal and non-verbal communication skills.
Skilled nursing facility experience preferred.
