Summary
The Health Advocate Case Manager plays a crucial role in providing comprehensive medical and case management services to individuals experiencing homelessness. This position involves assessing clients' healthcare needs, coordinating medical care, and providing advocacy and support to promote their overall well-being. The Health Advocate Case Manager works closely with interim housing staff, medical professionals, community organizations, and clients to ensure appropriate and timely access to healthcare resources.
Essential Functions
- Maintain a high level of ethical conduct regarding the confidentiality of employees, participants, company, and vendor data and handle sensitive and critical information with professionalism and discretion.
- Conduct initial assessments of clients' medical needs, including physical and mental health evaluations, and substance abuse history.
- Develop and implement individualized care plans in collaboration with clients to address their medical, mental health, and social service needs.
- Coordinate and facilitate access to healthcare services, including medical appointments, medication management, and referrals to specialists as necessary.
- Advocate for clients' healthcare rights and needs, collaborating with medical providers, social workers, and other relevant professionals.
- Provide education and support to clients regarding disease prevention, management of chronic conditions, medication adherence, and healthy lifestyle choices.
- Assist clients in navigating healthcare systems, including applying for health insurance, accessing prescription medications, and understanding healthcare benefits.
- Collaborate with interim housing staff and external agencies to ensure a holistic approach to client care, addressing both medical and social needs.
- Maintain accurate and up-to-date client records, including progress notes, treatment plans, and relevant documentation in compliance with confidentiality and regulatory requirements.
- Provide crisis intervention and emotional support to clients during medical emergencies or challenging situations.
- Participate in team meetings, case conferences, and staff training to enhance knowledge, skills, and collaboration.
- Maintain and uphold Agency's mission statement, values, policies, procedures, and principles.
- Maintain a safe environment at all times, complying with Agency and industry standards and safety regulations.
- Attend weekly care coordination conferencing meetings.
- Maintain up-to-date and accurate documentation in client files for service coordination/case management, housing placement, rental assistance, and move-in assistance.
- Ensure compliance with HIPAA, 42 CFR Part 2, and other funding requirements.
- Maintain appropriate boundaries; and adhere to SSG’s Code of Ethics and HOPICS’ Core Values.
- Represent the Agency in a professional manner at meetings, training, and community events.
- Regular attendance is required.
- Perform other duties as needed.
Secondary Functions
Overtime, holiday, or weekend work may be required.
Minimum Qualifications - Knowledge, Skills, and Abilities Required
- Bachelor's degree in social work, nursing, public health, or a related field and minimum two (2) years of job-related experience working with homeless individuals and families OR five years’ experience working in the social service field; case management and homeless program experience preferred.
- Previous experience in medical case management, preferably within a homeless shelter or similar setting.
- Strong knowledge of healthcare systems, resources, and community services related to homelessness, mental health, and substance abuse.
- Familiarity with medical terminology, common medical conditions, and healthcare best practices.
- Excellent communication skills, both verbal and written, with the ability to effectively interact with diverse populations and stakeholders.
- Ability to work collaboratively in a multidisciplinary team and maintain professional relationships with medical providers and community organizations.
- Empathy, compassion, and a non-judgmental approach to working with individuals experiencing homelessness.
- Strong organizational skills with the ability to manage multiple tasks and prioritize responsibilities effectively.
- Proficiency in using computer systems, electronic health records, and common software applications.
- Adherence to ethical standards, confidentiality, and regulatory requirements in healthcare and social service settings.
- If in recovery, a minimum of three (3) years of being drug and alcohol-free is required.
- Working knowledge of Microsoft Word, Excel, and other database programs. Knowledge of resource development, case management, and documentation.
- Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds.
- Ability to communicate effectively, both written and orally.
- Verification of Employment Eligibility and Background Clearance. TB test required (Not more than (3) months prior to or (7) days after the Date of Hire, and renewed annually thereafter), CPR and First Aid Certification required within 30 days of employment with the company.
Non-Essential Qualifications:
Experience using HMIS and at least one year of rapid re-housing experience preferred.
Supervisory Responsibilities:
This position does not have any supervisory responsibilities.
Environmental Conditions (Working Conditions)
This position is responsible to work in “client-friendly” environments and is required to visit other shelters and homeless access centers as part of their duties. Local automobile travel is required. There is some responsibility to work in noisy environments where children and adults are present.
Physical Requirements
The Case Manager typically spends time sitting, standing, typing, thinking, writing, walking, driving, carrying (max. 25 lbs.), listening, and speaking.
Mental Requirements
This position will require the individual to be able to handle any/all of the following: constant distractions, interruptions, uncontrollable changes in priorities/work schedules: be able to process information, think, and conceptualize.
