Southwest Community Health Center is Hiring a HIV Case Manager Near Bridgeport, CT

Summary: The Ryan White PART C Medical Case Manager role is to provide medical case management services to Southwest Community Health Center's patients, including, treatment adherence, psychosocial, education, referral, client centered services that link clients with health care, and crisis intervention.
SPECIFIC DUTIES AND RESPONSIBILITIES: All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA).
Demonstrates professional interdisciplinary skills and provides comprehensive Medical Case Management services to clients (including treatment adherence counseling).
Coordinates access to medically appropriate levels of health and support services and continuity of care
Develops initial/follow-up assessment of service needs.
Develops comprehensive, individualized service plan
Assists Infectious Disease and Internal Medicine department in data collection and fulfilling grant requirements.
Provides team support in Ryan White coordinator’s absence.
Follows established Ryan white Medical Case Management program and policies.
Orients new case managers to the Ryan White program and policies
Performs data entry as required by grants.
Attends assigned meetings and trainings, i.e., Medical Case management, staff, department, GBHCC, Emergency Fund Committee etc., as necessary
Performs other duties as assigned.
Qualifications:
Excellent writing, speaking and organizational abilities.
Cultural and ethnic sensitivity essential, ability to relate to people of diverse backgrounds
Must be a self-starter with the ability to work independently and as a team
Computer knowledge required
Some knowledge of substance abuse and crisis intervention techniques
Reliable vehicle to attend meetings/trainings required
Education and/or Experience:
Bachelor’s Degree in Human Services or three years related experience

Specific vision abilities required by this job include close vision, depth perception and the ability to adjust focus. Generally, the working conditions are good with little or no exposure to extremes in health, safety hazards and/or hazardous materials. This person must have the ability to travel as required to work with staff.
