Position: Case Manager

Job Summary:

We are seeking a dedicated and empathetic Case Manager to join our team at Soluna Wellness. As a Case Manager, you will play a vital role in supporting our clients' mental health journeys by providing comprehensive care coordination and ensuring the integration of services across various disciplines. Your primary responsibility will be to work closely with clients, their families, and other healthcare providers to develop and implement personalized care plans that address their unique needs and promote their overall well-being.

Responsibilities:

  • Conduct comprehensive assessments of clients' needs, strengths, and goals to develop individualized care plans in collaboration with the interdisciplinary team.
  • Coordinate and monitor the delivery of mental health services and support to clients, ensuring the integration of care across various providers and disciplines.
  • Serve as a primary point of contact for clients and their families, providing ongoing support, advocacy, and education regarding mental health resources, treatment options, and community support networks.
  • Collaborate with healthcare professionals, including psychiatrists, psychologists, therapists, and social workers, to ensure the continuity and quality of care.
  • Conduct regular client progress reviews, identify barriers to recovery, and develop strategies to overcome them in collaboration with the treatment team.
  • Assist clients in accessing community resources, such as housing, employment, education, and financial assistance, to enhance their overall well-being and promote recovery through individual and group sessions.
  • Maintain accurate and up-to-date client records, documenting all interactions, assessments, care plans, and progress notes in accordance with organizational policies and legal requirements.
  • Participate in team meetings, case conferences, and staff development activities to enhance knowledge and skills in the field of mental health and case management.
  • Stay current with emerging trends, research, and best practices in mental health care coordination, and apply this knowledge to improve the quality of services provided.
  • Uphold the ethical standards and professional code of conduct as outlined by relevant regulatory bodies and professional associations.

Qualifications:

  • Bachelor's degree in social work, psychology, counseling, or a related field. Master's degree is preferred.
  • Previous experience in case management or care coordination in a mental health or healthcare setting is highly desirable.
  • Strong understanding of mental health disorders, treatment modalities, and community resources.
  • Excellent communication and interpersonal skills to build rapport with clients, families, and healthcare professionals.
  • Ability to collaborate effectively with a multidisciplinary team and work in a fast-paced, dynamic environment.
  • Strong organizational and time management skills to prioritize and manage a diverse caseload.
  • Cultural sensitivity and the ability to work with individuals from diverse backgrounds.
  • Proficient computer skills, including experience with electronic health record systems and Google work systems.
