We are looking for an experienced RN Case Manager to join our growing hospice team!

Requirements:
Unrestricted UT RN License
Home Health Case Management Experience
Current CPR Certification
Reliable car and drivers license
Exceptional Clinical Skills in Hospice and End of Life Care
Exceptional communication, team oriented, efficient work style, and ability to complete timely documentation a must.
