We are looking for a MSW, LCSW to join our growing hospice team. 

Position is part-time covering Salt Lake County

Immediate opening start anytime between now and May 30. 

Requirements: 

Unrestricted UT LCSW License

Hospice Experience

Current CPR Certification

Reliable car and drivers license

Exceptional Clinical Skills in Hospice and End of Life Care

Exceptional communication, team oriented, efficient work style, and ability to complete timely documentation a must.
