Works under general supervision while carrying out the responsibilities of assessing, enrolling, and performing tasks to maintain enrollment of older and disabled persons in Home and community-based programs. The Program Coordinator assists persons in securing support and services that allow them to maintain health and functional abilities to reside in the community.
Pay Grade 48
Salary Commensurate with Experience and Education
1. Two (2) years of experience in geriatric case management, social work, or as an LPN; AND
2. Bachelor's degree from an accredited college or university in social work, sociology, nursing, or related field; OR
3. An equivalent combination of related education and/or experience.
4. PROOF OF EDUCATION, TRAINING, AND/OR EXPERIENCE IS REQUIRED.
1. Completes an assessment plan, a care plan, and a referral plan before enrolling consumers in HCBS Programs.
2. Plans and conducts social assessments for seniors and adults with disabilities seeking enrollment into Home and Community-Based Service Programs (HCBS).
3. Ensures a comprehensive screening, assessment, and evaluation of individual consumer’s health and functional abilities to ascertain the need for long-term care services.
4. Conducts follow-up reassessments and follow-up visits and provides service options to consumers in planning and rendering direct services.
5. Coordinates care plan compliance and service delivery for individuals eligible for Home and Community-Based Service Programs to prevent nursing home placement.
6. Works collaboratively with the staff of the Tennessee Commission on Aging and Disabilities, service providers, and other medical professionals to ensure individuals eligible to receive services are enrolled and served.
7. Ensures that an adequate number of consumers transition from referral to enrollment and subsequently receives services.
8. Assures case records remain current and accurate
9. Monitors all cases for the cost-of-service delivery dollar allocations, documents the cost for HCBS services, and ensures individual service costs do not exceed per-client program maximums.
10. Assists with compiling data and reporting.
11. Provides in-service training on the HCBS Program Systems as needed.
12. Performs other related duties as required or directed.

Must pass background check to include the following: Criminal History, Tennessee Abuse Registry, Tennessee Felony Offender Registry & the National and Tennessee Sexual Offender Registry.
