Job Title: RN - Case Manager (Home Health)

Location: Rochester, NY

Rate: $3200 Weekly

Shift: 40hrs - 5x8hrs - 8a - 4:30p - Days

Job Description:
The RN Case Manager will be responsible for coordinating and providing skilled nursing care to patients in their homes. This position requires a compassionate and detail-oriented individual who is dedicated to ensuring the highest level of care for our patients. The RN Case Manager will work closely with patients, families, physicians, and other healthcare professionals to develop and implement individualized care plans that promote optimal health outcomes and enhance the quality of life for our patients.

Responsibilities:

• Conduct initial assessments of patients' health status and develop comprehensive care plans tailored to meet their individual needs.
• Coordinate and provide skilled nursing care, including medication management, wound care, IV therapy, and other nursing interventions, in accordance with physician orders and established protocols.
• Monitor patients' progress and adjust care plans as needed to promote optimal health outcomes and ensure continuity of care.
• Educate patients and their families about their medical conditions, treatment options, and self-care techniques to promote independence and empower them to manage their health effectively.
• Collaborate with interdisciplinary healthcare team members, including physicians, therapists, social workers, and home health aides, to coordinate care and ensure the delivery of comprehensive services.
• Advocate for patients' needs and facilitate access to community resources and support services to enhance their overall well-being.
• Document all patient assessments, interventions, and outcomes accurately and timely in electronic health records (EHR) in compliance with regulatory requirements and agency policies.
• Participate in ongoing education and training to maintain current knowledge of best practices, healthcare trends, and regulatory changes relevant to home health nursing.
• Adhere to ethical and professional standards of nursing practice and demonstrate a commitment to excellence in patient care and customer service.
• Maintain open communication with patients, families, and team members to address concerns, answer questions, and ensure a positive care experience.

Requirements:

• Valid license to practice as a Registered Nurse (RN) in the state of New York.
• Minimum of two years of clinical nursing experience, preferably in home health, community health, or a related field.
• Strong assessment, critical thinking, and problem-solving skills.
• Excellent communication and interpersonal skills, with the ability to effectively collaborate with diverse individuals and teams.
• Compassionate, patient-centered approach to care with a commitment to delivering high-quality services.
• Ability to work independently and manage multiple priorities in a fast-paced environment.
• Proficiency in using electronic health record (EHR) systems and other technology tools.
• Valid driver's license and reliable transportation.
• BLS certification required; additional certifications such as ACLS or wound care certification preferred.
