Seven Healthcare are seeking an experienced Case Manager RN to join our team in Rochester, NY. The successful candidate will be responsible for managing the care and treatment of patients with complex medical needs. The Case Manager RN will work closely with patients, families, and healthcare providers to ensure that all aspects of care are coordinated and optimized.

*Responsibilities of the Registered Nurse Case Manager:*
* Conduct initial assessments to identify the needs and goals of clients
* Develop and implement comprehensive care plans, tailored to each client's specific situation
* Collaborate with a multidisciplinary team of professionals, including social workers, counselors, healthcare providers, and community resources
* Monitor and evaluate clients' progress towards achieving their goals on an ongoing basis
* Provide crisis intervention and support when necessary
*Requirements of the Registered Nurse Case Manager:*
* BLS Certification
* NY RN License
* 2 yearsCase manager experience
* Certified Home Health Experience
* OASIS experience
