Job Description:

• Must be licensed to practice medicine by the Kentucky State Board of Medical Licensure. Must be Board Eligible or Board Certified in the field of Psychiatry. Must hold a valid DEA license.
• This job requires mandatory COVID 19 vaccination or an approved exemption in accordance with CMS-3415-IFC from the Centers for Medicare & Medicaid Services. This means if appointed to the position, the applicant must have completed the primary vaccination series for COVID-19, which means the administration of a single-dose vaccine or the administration of all required doses of a multi-dose vaccine upon commencement of employment.The vaccine received must be one of the FDA-approved or FDA-authorized vaccines. Proof of vaccination will be required. If a medical or religious exemption is sought, the candidate must submit paperwork to the employer prior to appointment for review and approval.
