Position Summary: This position is responsible for providing end-of-life care and hospice services to patients in the hospice facility and may include providing care coordination, admissions facilitation, clinical support, continuous care, and/or other skilled nursing expertise with end-of-life care.
Qualifications Include:
Graduate of an associate’s, diploma, or baccalaureate nursing program
Current RN license by applicable state requirements; current BLS for Health Care Providers or will obtain within 3 months of start date
Daily access to a reliable and insured vehicle. Valid driver’s license with acceptable driving record per policy
Must have strong written and verbal communication skills, strong organizational skills, accurate and attentive to details, proficient keyboarding skills, prior experience with electronic medical record systems and belief in the hospice concept.
