*Job description:*
As a Care Coordinator at Samaritan Bethany, you will:
* Manage care of residents based on the resident-directed care plan
* Provide professional communication with residents’ interdisciplinary team
* Comprehensively assess, report changes, and track ongoing resident clinical needs
* Supervises direct care staff by providing direction, completing performance reviews and disciplinary action
* Make change to direct care staff schedule to ensure safe resident care
* Able to work in a fast paced environment and handle changes in daily tasks well regarding the resident care and provider follow-up.
* Critically analyze and make effective change that is best suited to benefit the residents care and staff tasks
*Requirements:*
* Must have a MN RN License and be in good standing
* CPR Certification
* Prior long-term care experience preferred
