Salary: $67,808 annually

Location: Victorian Heights

Schedule: Monday - Friday: 8:30 a.m. - 5 p.m. Candidate must be available to work up to 8 p.m. two evenings per week.

Under the supervision of Program Director the Transitional Aged Youth (TAY) Clinician will work in a team setting to provide Youth Systems Navigation (YSN) Outreach and Case Management to youth experiencing homelessness. Clinician will be responsible for providing mental health support to eligible youth and young adults that are experiencing homelessness or housing instability.. Clinician is committed to providing services that are trauma informed, strength based, person center and empowerment oriented.

Duties:

  • Provide mental health services including individual, family, psychoeducation, and intensive case management.
  • Provide case management services to youth experiencing homelessness.
  • Conduct client-screening interviews and assess service needs via phone and face-to-face interviews.
  • Assessment and case planning on each participant enrolled in the HOY program.
  • Provide quality trauma-informed and Positive Youth Development principles to participants.
  • Implement outreach to eligible youth.
  • Ongoing support and supervision of youth participating in programs.
  • Provide information and referrals to other program services.
  • Collect data, input case notes into county-monitored database, and maintain records and case files.
  • Provide educational and housing assistance to eligible youth.
  • Provide crisis intervention to clients as needed.
  • Provide assessment of competency and job readiness.
  • Assist in coordinating and facilitating workshops and special events.
  • Provide job placement assistance to eligible youth.
  • Provide advocacy and referrals based on clients’ needs.
  • Develop and maintain outreach and linkages in the community.
  • Collaborate with community-based organizations.
  • Attend all agency required meetings and trainings.
  • As other duties assigned.
Requirements:
  • Master's degree in social work, psychology or related degree required.
  • Current BBS registration as AMFT, ASW, or Associate LPCC for clinical candidates required.
  • Experience working with transition-age youth experiencing homelessness preferred.
  • Ability to work with confidential information.
  • Demonstrate initiative, resourcefulness, and problem-solving skills.
  • Adult role model with the ability to effectively set limits and function in crisis-oriented and high stress situations.
  • Ability to observe critical incidents and the physical ability to act swiftly in an emergency situation.
  • Ability to adequately observe participants’ activities and enforce safety regulations and apply appropriate policies and procedures.
  • Responsible for verbal and written reporting of contract goals and objectives on a monthly basis.
  • Ability to work with a diverse population, possess cultural sensitivity, and maintain appropriate boundaries with clients.

Conditions of Employment

  • Employment may be conditioned upon satisfactory completion of certain background clearances which may include a Department of Justice criminal background check requiring my fingerprints (Live Scan), screening through the National Sex Offender Public Website, drug test, tuberculosis (TB) test, and such additional clearances as may be required under the program.
  • Candidate is required to have reliable transportation; therefore, the candidate must successfully complete a motor history check. Candidate must also possess and maintain a current valid California driver’s license and valid auto insurance policy.

PHYSICAL DEMANDS:

  • Mentally and emotionally: Ability to work under deadlines, various projects and dealing with clients in crisis and difficult situations.
  • Must be able to remain in a stationary position during shift.
  • Moves throughout the building.
  • Moves, raises, or lowers (includes upward pulling) an object or equipment up to 50 lbs.
  • Transports an object, from 20 to 50 lbs. across the facility, for various needs.
  • Ability to exert force upon an object so that the object moves away from the force.
  • Ability to exert force upon an object so that the object moves toward the force (includes jerking)
  • Ascending or descending stairs and ladders when applicable. Body agility is emphasized.
  • Balancing: Maintaining body equilibrium to prevent falling when ascending, descending, traversing.
  • Stooping: Bending body downward and forward by bending spine at the waist, requiring full use of the lower extremities and back muscles.
  • Handling: Seizing, holding, grasping, turning, or otherwise working with objects or office machinery.
  • Communicating: Expressing or exchanging ideas. Communicating is important for those activities in which workers must impart information to clients or to the public, and in those activities in which they must convey detailed or important instructions to other workers accurately, or quickly.
