Sandy Park Development Center provides support for persons with developmental and intellectual disabilities by enhancing their quality of life. We do this through community involvement and participation as well as enrichment opportunities throughout their life. As a member of our staff, you will be a part of one of these living centers and provide vital around the clock care, which includes nursing and medical oversight, to our residents.

Purpose:

This position will complete the selection, purchase, maintenance and inventory of all personal effects to include clothing, shoes, personal care items, room decorations, and any other preferred items/activities as an advocate for residents’ financial interests. Adhere to all Accounting policies and procedures. Will act as contact person for the resident.

Essential Duties:

Act as contact person with local, state and federal agencies and as liaison to public and private service providers, including but not limited to Agency for Persons with Disabilities (APD), Medicaid, Medicare, Agency for Health Care Administration (AHCA), Social Security and Veteran Affairs. Complete all initial and annual applications for applicable funding sources.

Coordinate all admission and discharge documentation in accordance with guidelines and regulations.

Assist residents and families to participate in the development, understanding and implementation of Support Plans. Participate in developing and implementing Human Growth and Development skills as needed with our residents.

Plan and implement shopping trips for each resident based on need/ability. Ensure resident participation and choice in all purchases of personal items.

Assist and participate in resident recreational activities to include holiday festivities, resident birthday parties and any other seasonal or facility specific events.

Required Qualification:

A bachelor’s degree in the field of Human Services.

Valid Florda's driver's license and safe driving record.

Preferred Qualifications:

Long term care knowledge in Medicaid and Medicare benefits.

Bachelor's Degree in another field may be considered for candidates with five (5) years or more of related experience in the field of Social Services.

Salary/Hourly Rate:

$30.00 per hour
