Qualifications

B.A./B.S. in Psychology, Sociology; Social Work; or other degree.

An understanding of the psycho-social dynamic of the geriatric population and the ability to empathize and provide guidance to them

Experience as a SNF Social Services Director or Assistant with knowledge of hospital discharge planning and policies as it relates to Skilled Nursing Facilities

Knowledge of AHCA and CMS regulations relevant to social services, discharge procedures, and advanced directives in a SNF environment.

Responsibilities

Assist in the resident's adjustment to the facility and maintain the highest possible level of psycho-social functioning within the facility environment.

 Provide discharge planning to home or other appropriate facilities, including coordinating DME, transportation, home health care, and communication with families.

Greet new residents to the facility and orient them to the building and staff. Know and be known by every resident in the facility.

 Assess all new residents upon admission and complete the social service initial admission notes, social history, social assessment, and discharge planning when appropriate.

 Document quarterly and annual updates of Social Assessment and Social History

Complete the Social Service Section of the MDS form timely upon admission, quarterly, annually, and/or in cases of significant change 

Coordinate room changes including notifying families when necessary

 Manage the grievance program, address problems of residents and families by working with pertinent departments, and attempt to reconcile differences when possible

Obtain physician acknowledgments and signatures as required on documents and upload them to the resident electronic medical record.

Monitor psychiatric practitioner documentation and nursing notifications; update resident medical records for capacity and list of residents with elopement risk as needed.
