San Francisco Community Health Center is Hiring a Program Manager - Black Health Center of Excellence (BHCoE) Near San Francisco, CA

About the Program:

The Black Health Center of Excellence (BHCoE) serves African-American individuals living with HIV/AIDS who are accessing care at UCSF’s Ward 86 and MOCP. As a non-medical case management program, SFCHC’s BHCoE initiative aims to support enrolled clients with their medical engagement by providing strategic outreach and care linkage, strengths-based case management, resource navigation, treatment counseling, outreach and navigation, case consultation and coordination, and programmatic outreach and coordination. The BHCoE is part of a larger network of Centers of Excellence developed to support people living with HIV/AIDS and receives its funding through the Ryan White Care Act.

Position Summary:

The Program Manager will provide day-to-day oversight, supervision, and coordination of the BHCoE program, its two Case Managers, one Community Health Workers, and enrolled clients. The BHCoE Program Manager will report to the Associate Director, overseeing HIV treatment programs.

Essential Duties:

• Serve as day-to-day contact person for all program staff.• Coordinate all aspects of client care, client drop in, art therapy program, behavioral health groups and medical clinic supportive services.• Maintain and provide support in daily Drop In space.• Ensure that all direct service staff provide strengths-based, trauma-informed, and culturally competent services to clients.• Deliver or arrange training and resources required for direct reports and direct service staff to perform their roles.• Monitor and ensure program deliverables are being met.• Review program objectives and performance and design strategies, as needed, to improve service delivery.• Develop and maintain protocols to ensure client engagement in care and attending medical appointments.• Work with collaborating agencies to ensure strong partnerships are maintained.• Develop, maintain, and support schedule of SFCHC staff, including (1) requests for home visits/outreach needs and (2) open access programming.• Supervise, evaluate, and hire direct reports.• Support supervisees in identifying and reaching professional growth goals.• Assist the team with crisis management when there is a client crisis that arises.• Conduct outreach in various types of settings.• Assist with other administrative duties as needed.

Minimum Qualifications:

• Bachelor’s degree in health or related field. A combination of experience and education will be considered.• At least 1 year experience managing or supervising a team.• At least 2 years experience working with dually diagnosed unhoused/ housed individuals communities living with HIV.• At least 2 years experience with advocacy, counseling, referrals, or outreach services in the HIV field• Familiarity with issues and concerns of the LGBTQ community, sex workers and substance users; immigrants and refugees. Sensitivity to cultural and ethnic concerns as it relates to substance abuse, mental health, and HIV.• Ability to handle multiple tasks, work under pressure and to work professionally and ethically within multi-racial and multicultural settings; and appropriately seek out support.• Demonstrated excellence in attendance and reliability, interpersonal, organizational, verbal, and written communication skills, including attention to and respect for detail, accuracy, deadlines and follow up.• Intermediate Microsoft Office skills.• Knowledge of HIV/STD transmission and prevention. Knowledge of health and social issues related to HIV transmission in high-risk populations, specifically among African-American communities.• Demonstrated experience facilitating community groups/meetings.• Ability to work with and maintain boundaries with clients and confidential information.• Knowledge of legal mandates and ethical issues related to client privacy and confidentiality.• Strong organization skills and ability to apply new knowledge into day-to-day client work.• Self-directed, motivated, and flexible with the ability to work independently and as part of a team.• Ability to support the mission and goals of SFCHC.• Flexible schedule with ability to work weekends, evenings, and non-traditional work hours.• Must be able to sit, stand, and walk for extended periods of time.• Fully vaccinated against COVID-19 and able to show proof of vaccination and booster shot.• Up-to-date with vaccinations, especially seasonal flu shot vaccine.• Proof of TB clearance (issued within the past 12 months or less).

Preferred Qualifications:

• Knowledge and experience with the drivers and health disparities in the LGBTQ community and how they impact linkage and retention to HIV care services for HIV LGBTQ individuals.• Multicultural understanding.• 3 year of direct supervision experience.• 3 year of Case Management experience.• 3 years of experience with curriculum development.• Professional experience working with people living with HIV and those dealing with substance use and mental health complexities.
