San Francisco Community Health Center is Hiring a TACE Case Manager I Near San Francisco, CA

 The TACE (Tenderloin Area Center of Excellence) program aims to ensure and expand continuous seamless access to quality primary care and critical support services for “severe need” and “special populations” who are severely under-served residents of the Tenderloin. Targeted populations include homeless and marginally-housed residents of the Tenderloin, including residents who are active substance users, veterans, residents coping with mental illness, sex workers, transgender individuals, non-English speaking residents, gay males and their non-gay identified sex partners. The focus of TACE is to enhance HIV prevention services for members of these defined populations already living with HIV/AIDS who demonstrate a need for support in developing skills and behaviors to contain the spread of HIV/AIDS to their sexual and drug using partners.

SFCHC staff provide case management, care navigation, drop-in services, and, weekly groups. Tom Waddell Urban Health Center provides on-site psychiatric support and medical services. 

Position Description
The Case Manager Level I is responsible for providing psychosocial assessments, coordinating medical care plans with primary medical and behavioral health providers, health education, supportive counseling, crisis intervention, benefits advocacy, and coordinating services within their program as assigned by their direct supervisor. Direct client work and interaction will be done both in the field and at the office.

Essential Functions:

  • Intake and assessment of new clients
  • Case management and advocacy services for clients
  • Basic HIV treatment education and advocacy, as well as general health education
  • Supportive counseling and developing service goals with clients
  • Conduct individual risk reduction counseling sessions
  • Provide information and referrals to external services and monitor referral completion
  • Complete necessary documentation, UOS reports and treatment plans in a timely manner
  • Participate in biweekly case conferences, monthly staff meetings, and in-service training
  • Participation in and compliance with agency quality assurance, peer review, clinical consultation
  • Work with Care Services Team to identify new clients’ needs and develop programs accordingly
  • Navigate and outreach clients as needed
  • Perform other duties and tasks as needed

Minimum Qualifications:

  • BA in a health sciences or social work-related field or AAA/CMH/CHW OR comparable work experience
  • One year of experience providing counseling, advocacy, education and service coordination and/or outreach work to disenfranchised communities
  • Knowledge of HIV /AIDS, STIs, substance use, mental health, social service, and probation system
  • Familiarity with issues in communities of color
  • Knowledge of harm reduction philosophy, client centered counseling, and motivational interviewing techniques and trauma informed care.
  • Must be adaptive, flexible and a team player
  • Strong verbal and written proficiency in English
  • Experience working with diverse sexual/gender orientations, the homeless, limited English speakers, and/or multiple-diagnosed individuals
  • Demonstrated excellence in attendance and reliability, interpersonal, organizational, verbal, and written communication skills
  • Familiarity and experience with Microsoft Word and Outlook programs
  • Detail-oriented with strong follow-through skills, including attention to and respect for detail
  • Must be able to sit, stand, and walk for extended periods of time
  • Fully vaccinated against COVID-19, complete with booster shot(s) and able to show proof of vaccination 
  • Up-to-date with vaccinations, especially seasonal flu shot vaccine
  • Proof of TB clearance (issued within the past 12 months or less)
