San Francisco Community Health Center is Hiring a Prevention Coordinator Near San Francisco, CA

The Prevention Coordinator will work closely with the Program Manager of The Lotus Project and Stop the Hate program. The position will support to identify, develop, and coordinate HIV/HCV Testing events with primary focus on Asian & Pacific Islander (API) Men who have sex with Men (MSM), Transgender women, and others at risk for HIV/AIDS, Hepatitis C, STIs, substance use, behavioral health issues, marginally housed/homeless, and those who experience multiple barriers to primary medical care within The Lotus Project Program. The position will support the Stop the Hate program and the Direct Services Coordinator to develop and implement preventive anti-violence strategies to the LGBTQ API community.

This position is a part of The Lotus Project Program, a new innovative one-stop shop of health resources and services for the Asian & Pacific Islander community in close partnership with UCSF Alliance Health Project and the Stop the Hate (STH) program which implements anti-hate and anti-violence programming.

Program Planning and Implementation Essential Duties: 

HIV/HCV/STI Testing

  • With guidance from SFCHC’s Program Manager and partnership with UCSF Alliance Health Project, provide support to identify, design, produce, and conduct HIV/HCV/STI testing events offsite during mobile testing located at bars, clubs, or community-based street festivals with primary focus on Asian & Pacific Islander Men who have sex with men and Asian & Pacific Islander Trans women under the Lotus Project.
  • Conduct intake, assessment, and reassessment of clients.
  • Provide and support rapid HIV and/or Hepatitis C Testing.
  • Conduct site visits, monitor and maintain SFCHC’s Testing location(s) at 730 Polk Street and 1460 Pine Street.
  • Recruit, train, and supervise volunteer-based certified HIV/HCV Test Counselors
  • Assist multidisciplinary team in linkage to HIV/HCV/STI care services, navigation, support services, and case management for clients who test positive.
  • Collaborate and support other HIV prevention and treatment programs within SFCHC
  • Other duties as assigned.

Stop the Hate Program Essential Duties:

  • Work with the Direct Services Coordinator to lead and plan client prevention services.
  • Develop and oversee specialized anti-violence intervention and prevention services for Transgender API clients.
  • Implement anti-hate and anti-violence program within Trans Thrive drop-in center and the organization.
  • Develop collaboration with API Trans clients in the development, implementation and evaluation of project services.

Program Monitoring Essential Duties:

  • Complete and perform required data collection, and maintenance in a timely manner, including Program Update/Reports (monthly, quarterly, yearly) for workshop/special event, individual client education progress notes, group level and individual level interventions and track client enrollment/activities and contractual program documentation in accordance to standards as dictated by funding agency and deadlines.
  • Work in part of a team in developing and implementing process and outcome evaluations to measure program efficacy.
  • Provide support to staff and volunteer based certified test counselors who assist in program implementation and testing efforts of the organization.
  • Attend and actively engage in department meetings and various coordinating team meetings with partnering agencies
  • Implement client satisfaction surveys for on-going program improvement.
  • Co-supervise with Community Engagement Coordinator and Program Manager, the Lotus Project interns and volunteer-based peer leaders.
  • Support the development, implementation and facilitation of Lotus program Volunteer orientations, support groups and events.
  • Work closely with Program Manager and Community Engagement Coordinator on the continued improvement and implementation of the Lotus Project.
  • Work closely with the STH Program Manager and Direct Services Coordinator.
  • Supervise Community Engagement Coordinator and support implementation and facilitation of Lotus Program and STH support groups and social events.
  • Other duties and responsibilities as assigned.

Community Building and Advocacy Essential Duties: 

  • Develop SFCHC’s relationship with local community-based organizations, social venues, shelters, housing, and appropriate sub-groups of A&PI MSM and A&PI Transgender women.
  • Represent the agency and the community, and assume leadership in external collaborations
  • Promote the program and agency services in collaboration with community constituents and stakeholders
  • Participate and present the work of the department at local and national conferences, meetings in order to stay abreast of the developments in the sector, expand networks, and give visibility to the organization’s work in San Francisco and the larger region.
  • Develop collaborative relationships with allied social services agencies, to ensure and support client linkage and improve access to services.
  • Duties as assigned.

Minimum Qualifications:

  • Bachelor’s Degree in a health or social services related field. Combination of experience and education will be considered.
  • At least 3 years of experience in multi-level implementation of evidence-based interventions.
  • At least 2 years of experience in curriculum development and teaching in community-based settings
  • Extensive knowledge and experience with HIV/AIDS, substance use, mental health issues, among transgender communities.
  • Demonstrated experience in social service navigation, linkage/engagement in HIV medical care services, clinical service integration models
  • Extensive experience in develop program monitoring reports, contracts management, with strong attention to detail
  • Ability to work weekends, and evenings is required, and to be punctual and perform tasks consistently during scheduled shifts.
  • Strong and demonstrated experience in public speaking, communication, and writing skills
  • CA State Certified HIV/HCV Test Counselor
  • Comfortable discussing issues of sex, HIV/AIDS, as well as sexual, gender, and ethnic identities with transgender and GNC folks, youth, and adult men in various stages of development.
  • Strong written and verbal communication skills.
  • Strong attention to detail and demonstration of follow through and reliability.
  • Ability to work with a multi-disciplinary team.
  • Fully vaccinated against COVID-19, complete with booster shot(s) and able to show proof of vaccination 
  • Up-to-date with vaccinations, especially seasonal flu shot vaccine
  • Proof of TB clearance (issued within the past 12 months or less)

Preferred Qualifications:

  • Experience with CDC Evidence Based Interventions, especially Social Network Strategy (SNS) for Counseling, Test and Referral (CTR) and Popular Opinion Leader (POL)
  • Experience with software: Microsoft Office suites, Google suites, Canva, etc.
  • Possess an active phlebotomy certification
  • Reliable means of transportation
