Description

About the Role:

Activity Coordinator to join our team and play a crucial role in developing and implementing engaging activities for adults with intellectual and developmental disabilities. Our goal is to make our programs fun, integrated, and engaging for people with disabilities to feel fully a part of their local communities. This role will be responsible for meeting with each regional office location to develop activity schedules alongside input from people in services to get us involved in community activities that are meaningful to each person.

Key Responsibilities:

  • Work collaboratively with individuals in services to identify and choose activities based on their interests, preferences, and developmental goals.
  • Facilitate regular meetings and discussions to gather input and feedback from program participants, ensuring that activities align with their unique needs and aspirations.
  • Create a supportive environment that empowers individuals to express their choices and preferences in activity selection, fostering a sense of autonomy and self-determination.
  • Develop and maintain partnerships with local organizations, businesses, and community members to establish volunteer opportunities for program participants. Coordinate and oversee these opportunities to enhance skill development and community engagement.
  • Cultivate meaningful community connections across various locations in Colorado by organizing and participating in community events, workshops, and social activities. Actively seek out opportunities to promote inclusion and interaction between individuals with intellectual and developmental disabilities and the broader community.
  • Collaborate with internal teams to integrate therapeutic and educational elements into activities, ensuring that each program participant receives personalized support that aligns with their individualized support plans.
  • Maintain accurate records of activity participation, individual progress, and community connections. Regularly assess the effectiveness of activities and make adjustments to continuously improve the overall program.
  • Foster a positive and inclusive atmosphere within the day habilitation programs, promoting a sense of belonging and well-being among program participants.
  • Works closely in coordination with IDT teams including, but not limited to case managers, guardians, advocates, state personnel, therapists and families to monitor the adult in placement
  • Completes ongoing audits of the provider and client files, and ensures that all documentation is in compliance with CO Developmental Disabilities Standards
  • Participation in relevant meetings for each adult on caseload to ensure all needs are being advocated for appropriately and that rights are being protected
  • Advocates for the development of services/supports needed for people with developmental disabilities
  • Available to provide backup services for vacations or other prolonged absences of other team members
  • Works in full compliance with all of Sample Supports’ policies, procedures and protocols
  • Job responsibilities include reporting any suspected maltreatment, abuse, neglect, or exploitation of an at-risk individual. (M/A/N/E) Each position is considered a mandatory reporter
  • Willing and able to provide direct services to individuals receiving services from Sample Supports, if requested
  • Maintains reliable transportation to and from work and to transport clients
  • Expected to cover shifts as needed to maintain expected billable hours in the event they are requested
  • Any other tasks and duties as assigned

Requirements

The ideal candidate possesses the following characteristics/skills:

  • Experience in working with individuals with intellectual/developmental disabilities and/or human services in a social service setting
  • Capable of meeting the physical demands of the position
  • Must be 18 years of age or older due to driving requirements on behalf of the organization
  • Must maintain an active Driver's License and have a reliable vehicle for transportation of individuals receiving services, as needed
  • Successful completion and maintenance of certifications in CPR, First Aid, Medication Administration, NVCI, and any other required or requested training by the organization
