Position Overview:
We are currently seeking a skilled and experienced Nurse to join our team of healthcare professionals. The Skilled Nurse will play a crucial role in delivering comprehensive nursing care to our clients, promoting optimal health outcomes and enhancing their overall well-being. Responsibilities:

• Conduct comprehensive assessments of patients' health status, including physical, emotional, and psychosocial needs.
• Develop individualized care plans in collaboration with the interdisciplinary team, tailored to meet the unique needs and preferences of each client.
• Provide skilled nursing care, including wound care, medication management, infusion therapy, and other specialized treatments, while ensuring patient comfort and safety.
• Monitor and evaluate patients' progress, documenting observations, interventions, and outcomes accurately and timely.
• Educate patients and their families on health maintenance, disease management, and self-care strategies, empowering them to actively participate in their care.
• Coordinate with physicians, therapists, and other healthcare professionals to ensure continuity of care and optimize treatment outcomes.
• Advocate for patients' rights and preferences, promoting a culture of respect, dignity, and autonomy.
• Collaborate with community resources and healthcare agencies to facilitate transitions between healthcare settings and ensure comprehensive support for clients and their families.
• Adhere to regulatory requirements, agency policies, and standards of practice governing home health care services.
• Participate in ongoing education and professional development activities to enhance clinical skills and stay abreast of advances in nursing practice.

Qualifications:

• Valid and current licensure as a Registered Nurse (RN) in the state of Michigan.
• Minimum 1-2 years of nursing experience, with previous experience in home health care preferred.
• Demonstrated proficiency in clinical nursing skills, including wound care, medication administration, and infusion therapy.
• Strong assessment, critical thinking, and problem-solving skills.
• Excellent interpersonal and communication skills, with the ability to establish rapport with patients and families.
• Compassionate and empathetic attitude towards clients and their families, with a commitment to delivering person-centered care.
• Reliable transportation and willingness to travel to clients' homes within the designated service area.
• CPR certification and other relevant certifications may be required.

Job Types: Full-time, Part-time

Pay: $35.18 - $40.53 per hour

Expected hours: No less than 40 per week
