What we're looking for:The Behavioral Health Home (BHH) Nurse Care Manager will work as part of an interdisciplinary team to provide community-based integrated physical and behavioral health services to individuals who may have complex and chronic mental illness, substance abuse, and medical comorbidity. This position will work with individuals served to identify chronic behavior, discuss impact, and develop improvement strategies and specific goal-directed interventions. Compensation Range:The starting pay for this position is $62,613.13 - $73,662.50 annual salary, which is based on non-discriminatory factors such as skills and experience. Works within a team caseload of individuals with co-occurring physical and mental health needs
Meets regularly with individuals served in office and community settings
Participates in the selection of strategies to implement evidence-based wellness and prevention initiatives
Participates in initial care plan development including specific goals for all enrollees
Communicates with medical providers, subspecialty providers including mental health and substance abuse service providers, long-term care and hospitals regarding records including admission/discharge
Provides education in health conditions, treatment recommendation, medications, and strategies to implement care plan goals including both clinical and non-clinical needs
Monitors assessments and screenings to assure findings are integrated in the care plan
Collects accurate vital signs as part of assessment/ongoing monitoring Including height, weight, BMI, waist circumference, blood pressure, pulse, respirations, pulse oxygen, glucose and CO
Completes Point of Care testing when indicated for ongoing monitoring of psychiatric, substance use and/or medical conditions. Tests may include: HA1C, Lipids, WBC, Urine Pregnancy test, and Urine Drug Screen
Assesses and recommends laboratory testing based on an individual’s medications, psychiatric and medical conditions
Facilitates the use of the Electronic Health Record (EHR) and other Health Information Technology (HIT) to link services, facilitate communication among team members and provide feedback
Meets regularly with the care team to plan care and discuss cases and exchanges appropriate information with team members in an informal manner as part of the daily routine of the clinic
Identifies current chronic medical condition trends
Assesses barriers to care and implements solutions
Identifies health care promotion and prevention services
Attends Care Team Meetings
Completes all required documentation completely, accurately, and in accordance with CNS policy
Assesses and monitors the health status of assigned individuals
Assists in the coordination and monitoring of medication for individuals
Seeks out, links, and collaborates with primary and specialty care physicians, community health agencies, and other health care providers
Administers medications
Provides education and monitoring for adverse reactions to medication
Establishes a collaborative relationship with prescribers and clinic staff
Facilitates weekly persons served doctor appointments
Other duties as assigned
Qualifications:Required
Current unrestricted RN license
Valid and unrestricted Driver’s License
Preferred
Bachelor of Science in Nursing (BSN) degree
One-year of experience (Full-Time) working with adults in a mental health setting
