POSITION SUMMARY:

The Case Management Coordinator works as a member of the Site Management and Student Services Department team. Primarily responsible for overseeing all aspects of case management training, audit compliance and outside agency relationships, including community service, to ensure treatment continuity is maintained for the students. Depending on location, the Case Management Coordinator reports to the Director of Student Services or Clinical Director. May supervise the Case Management Assistant or other designated staff and directly or co-supervise Case Managers.

ESSENTIAL FUNCTIONS:

  1. Acts as a liaison for Rite of Passage with Placing Agencies and Licensing Officials. Makes periodic visits to Placing Agencies to build positive relationships.
  2. Reviews, audits and edits all case management job functions and reports (i.e. files, treatment groups, Positive Skill Development Group Meetings, visits, monthly and quarterly reports, CIR and runaway plans, etc.) and provides oral and written documentation of any findings.
  3. Creates, implements and maintains a functional student file system, if required. Conducts quarterly student file audits.
  4. Assists with developing supervised staff by conducting training sessions and action plans, if applicable. Schedules, oversees and provides required pre-service and on-going training for all new case management employees to ensure compliance with licensing and site standards. Provides direction to Case Managers regarding individualized programs. (This function depends on location).
  5. Manages student movements – progressions, regressions, admissions and exits.
  6. Assists in the coordination of mental health services.
  7. Ensures treatment classes are implemented as designed.
  8. Creates treatment plans in collaboration with the contracted psychologist.
  9. Creates and audits orientation files and requests missing documentation, if required.
  10. Monitors, evaluates and corrects the daily performance of the case management report, if required.
  11. Evaluates family visits.
  12. Reviews and responds to student grievances, when necessary.
  13. Responds to concerns from Placing Agencies regarding documentation and communication.
  14. Ensures Case Management compliance with licensing and certification standards. Analyzes program elements and provides feedback to supervisor to ensure program standards remain the highest in the industry.
  15. Assists with hiring, evaluations, commendations, reprimands and dismissals regarding supervised staff, if applicable.
  16. Reviews and approves time and attendance for supervised staff, if applicable.
  17. Completes required reports and documentation in a timely manner, provides management with required reports and advises of any problematic situations. Completes the weekly case management coordinator report, if required.
  18. Complies with and implements the Rite of Passage Policies and Procedures as detailed in the appropriate manuals/handbooks. Ensures the highest standards are maintained to prevent illegal, unethical, or improper conduct and to ensure the program remains in compliance with agency licensing and Rite of Passage policies and procedures.
  19. Assists with the correction of deficiencies and quality improvement efforts.
  20. Attends and participates in all required meetings. (i.e. site Treatment Committee meetings and monthly Student Services meetings). Facilitates weekly unit manager/case manager meetings, if required.
  21. Commits to attending and tracking all training and staff development classes in order to ensure sufficient hours of training for this position and all supervised staff are completed on an annual basis. Notifies the supervisor if annual training hours are deficient.
  22. Ensures the safety, health and welfare of staff and students at all times.
  23. Provides encouragement, guidance and resources to staff and students when needed.
  24. Models and ensures all program norms are upheld without compromise.
  25. Acts as a positive role model and mentor for both staff and students.
  26. Treats others with respect, confronts negative behavior and supports confrontations.
  27. Other duties as assigned, verbally or in written form.

MINIMUM QUALIFICATIONS:

  1. Bachelor’s degree in related field. Master’s degree preferred.
  2. Four years of experience in Human Services with at least two years of Rite of Passage experience.
  3. Prior case management experience (Rite of Passage experience preferred).
  4. Supervisory experience preferred.
  5. Knowledge of Placing Agencies, licensing requirements and Rite of Passage policies and procedures.
  6. Good interpersonal skills including the ability to interview potential employees if required.
  7. Ability to be an eligible ROP driver. Must possess a current State Driver’s License and have an acceptable driving record for the past three (3) years.
  8. Strong knowledge of overall company operations and policies and procedures.
  9. Ability to pass a criminal background clearance check, drug screen, physical and TB test.
  10. Ability to perform work with little or no supervision.
  11. Ability to utilize resources available to complete assigned projects.
  12. Ability to prepare written reports and correspondence.
  13. Ability to understand and follow verbal and written instructions.
  14. Ability to effectively communicate, verbally and in writing.
  15. Able to work in excess of 40 hours per week with the possibility of a varied schedule.
  16. Must be able to maintain a high level of confidentiality.
  17. Must have excellent organization and time management skills.
  18. Ability to build and maintain positive internal and external relationships.
  19. Ability to provide exemplary customer service to all employees and outside constituents.
  20. Ability to function independently and as a member of a team in a multi-task environment.
  21. Must be flexible and able to handle multiple priorities, with the ability to adjust to high pressure and rapidly changing business conditions.
  22. Proficient in the use of computers and associated software.
