Position: Care Representative I - Full Time

Availability: Immediate

Reports to: Site Manager

Rincon Family Services (RFS) is a not-for-profit community organization, established in 1972 to combat the epidemic use of heroin and violence that were dismantling communities throughout Chicago. RFS is dedicated to rendering compassionate and effective programs that promote the quality of life by serving as a catalyst to advocate, educate, and empower individuals and their families.

POSITION SUMMARY: The care representative will work in a service environment as a member of a multi-disciplinary team as the first point of contact for greeting and engaging with clients and visitors and will perform a full range of administrative tasks that are relevant to promoting quality delivery of services.

RESPONSIBILITY: Under the direct supervision of Site Manager, the Care Representative I carry the responsibility to offer clients the guidance and support needed to receive services at Rincon Family Services. Care Representative I also provide administrative support across the organization.

ESSENTIAL DUTIES: TheCare Representative I must demonstrate the ability toperform the following:

  1. Welcome and greet clients and visitors, and provide appropriate guidance, ensuring all guests understand and know the basic expectations for their visit and that all staff are notified as needed.

  1. Open or close the building as required, including and with respect to special events or activities.

  1. Answer, transfer and/or take messages from incoming calls, as well as reviewing voicemails every morning and forwarding information appropriately.

  1. Communicate via email and/or Microsoft TEAMS, as needed.

  1. Keep updated information about programs throughout the agency, especially at designated sites.

  1. Receive mail and packages, working in collaboration with the Site Manager for appropriate distribution and/or inventory.

  1. Care for aesthetics of the building, including but not limited to ensuring appropriate signage, watering plants, and making any facilities requests, as needed.

  1. Take and enter payments into the deposit log and EMR systems.

  1. In collaboration with Care Representative II and program leads, schedule clients for initial appointments.

  1. Maintain and file program records, through both paper and electronic medical records, meeting documentation standards as required by program and funding sources.

  1. Make copies, charts and send faxes for administrative and programmatic requests, as needed.

  1. Attend all required meetings, committees, and trainings according to program standards.

  1. Assist Site Manager to maintain program and building compliance in accordance with health and safety, HR, and OSHA standards.

  1. Help prepare sites and programs to be audit compliant.

  1. In collaboration with Care Rep II, assist programs with re-engaging consumers at risk of discharge and/or recent discharges to gain consumer feedback on programs.

  1. Participate in quality assurance and quality improvement activities as directed.

  1. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to Compliance policies and agreements regarding confidentiality, privacy, and security.

  1. All other duties as assigned.

METRICS

  • Work to gain 90% satisfaction from all guests on guest satisfaction survey results throughout the year, specific to front desk interactions.

  • Ensure that 95% of all calls to the main front desk line are answered.

  • Ensure that 100% of voicemails are answered and directed appropriately.

  • Provide a clear schedule of daily operation activities, including open/close of building.

  • Successful re-engagement of at least 15% of consumers contacted.

PHYSICAL DEMANDS:

The Care Representative must be able to successfully perform essential functions of this job, including carry charts to and from their offices/desks, prepare for group activities that may entail moving chairs and/or tables, carrying bags/boxes of food or decorating group activity areas. WORK ENVIRONMENT: The work environment characteristics are representative of those an employee encounters while performing the essential functions of this job. There may be occasions that call for an employee to carry out duties at other agency offices.

MINIMUM QUALIFICATIONS: The Care Representative must meet the following qualifications:

  • Minimum High School Diploma or equivalent required.

  • Demonstration of skills in MS Office (Word, Excel, PowerPoint, Access, Outlook).

  • Knowledge of Computer Systems (Internet, Email, MS Word, Excel, SAMMS, CIS, or similar preferred)
