Work in the Coordinated Entry System within HMIS to draw names of potential program participants and match their needs to Renaissance’s housing programs.
Job duties include but are not limited to:
Working within HMIS and the Coordinated Entry System to pull names of disabled and homeless participants for openings throughout the agency.
Ensuring proper documentation of homelessness and disability following requirements of HUD CoC guidelines.
Collaborate and communicate with Team Leaders and Program Directors about the intake and qualification process.
Aiding potential program participants with obtaining documentation from other organizations.
Complete Clinical Intake Assessment with program participants and entering all information into the organizations Electronic Health Record (EHR).
Coordinate with the Housing Department when a program participant is ready for housing.
Supplying information, linkage, referral, and advocacy for potential program participants if needed.
Maintaining the confidentiality of client information in any written, oral, or electronic form including, but not limited to, health information, HIV/AIDS status, and disability information. Releasing such information only as needed and with the proper release from the client unless mandated by law.
Must have the ability to relate appropriately and effectively with residents and families, to other staff and to personnel of other community agencies; knowledge of and access to community resources, instructional skills to present information; skill to organize and document notes and service plans; knowledge of and willingness to match programs and services appropriate for clients and residents; skill to establish and maintain effective working relationships; ability to work independently and as a member of an interdisciplinary team including residents.
Minimum of a bachelor’s degree in social work or a related field and two years of recent experience with lowincome families in a social services capacity.
