The RN Case Manager provides a combination of direct care by performing nursing visits, overseeing and directing healthcare services for clients, and collaborating with the client, their family, and entire healthcare team.

Major Responsibilities:

Represent the company’s values while maintaining a professional working relationship with others. Perform assessments and evaluation of effectiveness of goals. Advocate for the clients and ensure that they are in a safe environment. Follow all guidelines and stay in compliance with documentation, supervision, and policies. Provide skilled visits for clients and provide education to clients, family, and caregivers. Independently manage the work week and remain flexible in achieving all tasks and adapting to unexpected changes. Supervise unskilled caregivers and provide oversight and education. Provide overall care coordination and play an active role in the client’s healthcare team.

Schedule:

· Full time, flexible schedule

· Monday-Friday, no weekends, or holidays

RN Case Manager Requirements:

• Must have a current MN RN nursing license.
• Must be CPR certified.
• Must have great leadership skills and ability to provide education.
• Must have great communication skills and the ability to build and maintain professional relationships.
• Must be able to pay attention to detail and demonstrate critical thinking skills.
• Must be able to work independently and prioritize with visits, tasks, and deadlines.
• Must have basic computer skills with the wiliness to learn new EMR software.
