Job Title: Social Worker - HospiceJob Summary:The Social Worker - Hospice is responsible for providing emotional support, counseling, and assistance to terminally ill patients and their families. They work closely with medical teams to help improve the quality of life for patients during their end-of-life care.Responsibilities and Duties:1. Conduct psychosocial assessments of patients and families to determine their needs.2. Develop individualized care plans and goals for each patient and family.3. Provide counseling and emotional support to patients and families dealing with grief and loss.4. Coordinate with medical professionals, caregivers, and community resources to provide comprehensive care.5. Advocate for patients' rights and preferences in accordance with their wishes.6. Facilitate communication and decision-making within families regarding end-of-life care.7. Assist with end-of-life planning, including advance directives and funeral arrangements.8. Collaborate with interdisciplinary team members to address the spiritual, cultural, and social needs of patients and families.Qualifications and Skills:1. Bachelor's or Master's degree in social work from an accredited institution.2. Licensure as a social worker in the state of practice.3. Experience working with terminally ill patients and their families preferred.4. Strong communication and interpersonal skills.5. Ability to provide compassionate and empathetic care.6. Knowledge of community resources and support services.7. Ability to work independently and as part of a team.8.
