JOB TITLE: Social Worker LCSW – Cancer Center/BH
DEPARTMENT: Cancer Center/Behavioral Health
REPORTS TO: Service Line Director/ BH Clinical Supervisor
Job SummaryThe Social Worker develops and maintains an outpatient caseload within their clinical scope of practice. Scope of practice must include children and adults. In addition to this work at the Behavioral Health Clinic, the LCSW will provide services at the Cancer Center for Cancer Center Patients. This position is expected to provide 10 hours per week for the Behavioral Health Clinic and 30 hours per week for the Cancer Center.
The Social Worker staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served. The Social Worker must be able to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to his or her age-specific needs, and to provide the care needed as described in the Clinic’s policies and procedures.
Job RequirementsMust have LCSW license or provisional clinical social worker license. Master’s degree or higher in Social Work or equivalent degree with LCSW/PCSW licensure.
ExperienceBackground in general mental health services. Experience with clinical and medical social work preferred.
SkillsAbility to do: 1) problem assessment, 2)mental status evaluation, 3)psychosocial history interview, 4)diagnosis based on current DSM-V criteria, 5) treatment planning, 6) treatment evaluation, 7) psychological evaluation.
Required CertificationsLicensed in Wyoming as a LCSW/PCSW or licensed in another state as an LCSW/PCSW, and eligible for licensure in WY as an LCSW/PCSW.
Essential FunctionsGeneral
Provide an average of 95 billable hours of service each month (1144 hours per year).
Behavioral Health
  • Design appropriate treatment plans for individual, couple and group therapy.
  • Deliver a variety of psychotherapies including supportive and cognitive interventions.
  • Provide Medication Assisted Treatment (MAT), Intensive Outpatient (IOP), individual and group mental health/substance use therapy in coordination with the MAT/IOP treatment team.
  • Complete Intake Assessments with new clients.
  • Design appropriate treatment plans based on assessment for individual, SUD/MH group therapy. May also include couple/family therapy.
  • Deliver a variety of therapeutic modalities including supportive and cognitive interventions, individual therapy, group therapy, and family therapy (as needed).
  • Assist in program development for the Behavioral Health Clinic/Cancer Center.
  • Provide mental health services for physician referrals.
  • Maintain regular and timely communication with all members of the multidisciplinary team to ensure proper and timely care.
  • Maintain thorough and up to date progress notes on all patients assigned.
  • Maintain all CARF standards that apply to the Behavioral Health Clinic.
  • Formalize and complete aftercare plan to assure ongoing patient recovery on all patients assigned regardless of length of treatment and/or completion of treatment.
  • Formalize and complete discharge summary for each patient assigned.
  • Conduct community education sessions and lectures as assigned.
  • Serve as group therapist as assigned.
  • Conduct regular individual therapy sessions as scheduled on all patients assigned.
  • Complete assessments and evaluations on individuals referred through court, schools, attorney, etc., and provide written reports for the referent.
  • Be familiar with and comply with all federal and state regulations regarding confidentiality and reporting instances of abuse.
  • Ensure patient care is delivered in a safe and effective manner in compliance with state, and federal requirements.
  • Coordinate with all staff and aftercare referral agencies to ensure smooth and effective discharge of all patients assigned.
Cancer Center
  • Provide psychosocial assessments for each new Cancer Center patient.
  • Attend biweekly (every 2 weeks) Interdisciplinary Team Meetings. Provide information regarding conditions affecting the patient and family adjustment and coping. Update the plan of care according to the patient and family needs as well as goals and outcomes.
  • Treatment Plan: Establish a treatment plan with evaluations and revisions as indicated to accommodate changing patient/family needs. Establish visits and follow up care as patients and family’s needs arise.
  • Crisis Intervention, Counseling, Anticipatory Grief, and Loss Interventions: Provide supportive counseling services to the patient/family experiencing emotional, social, or economic conflict related to the illness.
  • Provide education regarding grief and loss.
  • Consult with nursing staff regarding patient care issues.
  • Provide services necessary to resolve social or emotional problems that are or expected to be an impediment to the effective treatment of the patient’s medical condition or rate of recovery and are indicated on the plan of care.
  • Provide counseling Services that are required by the patient and brief counseling (two to three visits) of the patient’s family member or caregiver when such counseling is needed to remove clear impediments to the effective treatment of the patient’s medical condition or to the rate of recovery.
  • Assist patient and family with community resource linkage, including financial and psychosocial resource needs.
  • Document via visit notes, summaries of care provided and discharge summaries on a timely basis.
  • Develop and lead a caregiver support group for the Cancer Center.
Secondary Functions
  • Speak as requested for community presentations and hospital-based support groups.
  • Conduct outside speaking presentations to schools, civic organizations, churches, etc.
  • Assist in prevention programs as assigned.
  • Serve on designated committees as required.
  • May be responsible to perform other duties and responsibilities for which the individual is qualified.
