Mental Health Practitioner Vocational Specialist

FACT is a service delivery model intended for individuals with serious mental illness who are involved with the criminal justice system. These individuals may have co-occurring substance use and physical health disorders. Their needs are often complex, and their disorders are often under-managed and further complicated by varying degrees of involvement with the criminal justice system. FACT builds on the evidence-based Assertive Community Treatment (ACT) model by making adaptations based on criminal justice issues - in particular, addressing criminogenic risks and needs. In this sense, FACT is an intervention that bridges the behavioral health and criminal justice systems. FACT is designed to do the following: improve clients' mental health outcomes and daily functioning; reduce recidivism by addressing criminogenic risks and needs; divert individuals in need of treatment away from the criminal justice system; manage costs by reducing re-occurring arrest, incarceration, and hospitalization, and increase public safety.

The Mental Health Practitioner Vocational Specialist will use an Individual Placement and Support (IPS) model to help people find, get and stay involved in educational or employment opportunities. Training in the model is available. Using this lens, the Vocational Specialist integrates trauma informed, clinical, and psychiatric rehabilitation interventions. They provide cross trainings and guidance to the team to facilitate the team's collective effort to support achievement of vocational goals. Additional job responsibilities include helping people access and maintain benefits and community resources, providing care coordination with external providers, connecting with natural support and providing crisis intervention services.

The schedule is Mon-Fri 10am-6pm. The position participates in an on-call rotation every 5-8 weeks that entails being available to respond to phone calls after hours for crisis intervention.


Qualifications:

  • Bachelors or Masters Degree in Behavioral Sciences or related field
  • 2,000 hours experience working with individuals with Mental Illness
  • Interest and/or experience in vocational rehabilitation and supported employment
  • Preferred: knowledge of forensic evidenced-based practices and familiarity with criminal justice system

Duties and Responsibilities:

  • Assist clients in seeking competitive employment in the community.
  • Coach clients with job interview skills and assist with paperwork.
  • Work with the team to provide FACT services for clients, assume primary responsibility for developing, writing, implementing, evaluating and revising overall recovery goals and plans in collaboration with the client and the treatment team per TMACT and FACT fidelity.
  • Facilitate development of comprehensive assessments, individualized treatment plans, substance abuse treatment, and other tools necessary to promote recovery.
  • Work with clients on all aspects of psychiatric rehabilitation, support, and psychotherapeutic interventions identified in the treatment plan, including Medication Assistance and Support.
  • Will provide coaching and education on independent living skills such as cooking, shopping, cleaning, personal hygiene, transportation, budgeting, organization, and housing.
  • Train other team members about supported employment services using the Individual Placement and Support Model. 
  • Collaborate with FACT nurse and health care providers to promote the client's physical and dental wellness goals. 
  • Attend commitment and other court hearings as required. Complete required court reports, and testify when requested.
  • File reports with Vulnerable Adult, Child Protection, the Ombudsman’s Office, and other mandated/required reports when applicable.
  • Collaborate with the Substance Abuse Specialist in assessing, planning and treating substance abuse for clients with co-occurring disorders in individual or group settings.
  • Work within a multidisciplinary team consisting of psychiatrist, team leader, mental health professionals and practitioners, certified peer specialist, nurses, vocational specialist, substance abuse specialist, team coordinator, and supervising agent.
  • Actively and assertively engage with the clients and their family members/significant others, including landlords and employers (with client approval).
  • Provide on-call crisis intervention covering evening hours and serve as a backup to evening and weekend staff on a rotating schedule.
  • Available to work weekend, evening, and holidays as needed.
  • Attend and participate in scheduled team meetings.
  • Provide transportation when applicable to clients' treatment plan goals.
  • Promptly respond to email, text, and telephone communication. 
  • Complete all documentation, reports, and charting contemporaneously and in an organized manner per FACT policy and procedure manual. 
  • Participate actively in on-going professional growth and development; maintain appropriate professional behavior and participate in supervision as determined by supervisor.
  • Actively participate in the coordination and discharge planning with clients for IRTS, hospital, jail/prison, treatment, and other community services
  • Maintain a pattern of regular work hours.
  • Other duties as assigned.

CEU Requirements: 

  • 30 Continuing Education (CEU) Credits every 2 years.

Competencies:

  • Knowledge of symptoms and course of serious and persistent mental illness as well as medications and other therapies.
  • Knowledge of current principles, practices and techniques of case management.
  • Knowledge of community resources.
  • Be knowledgeable about and utilizevarious chemical health treatment modalities such as harm reduction and motivational interviewing.
  • Have knowledge of evidence-based clinical practices and empirically-supported treatments.
  • Have demonstrable working knowledge of vulnerable adult laws and other pertinent statutes and regulations.
  • Knowledge of supportive counseling techniques, independent living skills/teaching and crisis intervention techniques.
  • Assess and manage crisis situations.
  • Utilizes crisis and risk assessment tools for suicidal and aggressive behavior.
  • Develop and maintain appropriate therapeutic relationship with clients.
  • Ability to navigate electronic healthcare record, and other computer software. 
  • Able to problem solve using analytical skills, creativity and logical thinking to reach a solution.
  • Able to work under pressure in a fast paced time sensitive environment with shifting priorities and multiple deadlines.
  • Multitask in an environment with multiple distractions is a must. 
  • Must have good interpersonal skills and ability to work independently and be part of an FACT team. 
  • Be able to speak, clearly understand, and write the English language.
  • Must have a car, a current driver’s license, valid auto insurance and be able to drive during the day or evening hours that may require you to drive in the dark.
  • Be able to accommodate flexible working hours.
