Reports to the Nurse Manager / Nursing Supervisor
Summary: The licensed practical nurse (LPN) functions under the direct supervision and guidance of the Nurse Manager /Nursing Supervisor. LPNs perform tasks and responsibilities within the framework of case finding, health teaching, health counseling and provide supportive and restorative care under the direction of a registered professional nurse (RN), clinical nurse specialist, physician, nurse practitioner, licensed midwife, physician assistant, specialist assistant, dentist or podiatrist. An LPN typically provides the following types services under the direction of an RN or other qualified practitioner The Licensed Practical Nurse participates in the multi-disciplinary process of providing care and treatment to residents. The position is expected to provide clinical support and technical assistance to the members of the interdisciplinary team. The LPN is responsible for functioning within the scope of practice dictated by the authority of the New York State Department of Education. Individuals must be able to manage demanding workload with accuracy. Position requires excellent customer service skills with residents families, staff, physicians, vendors and the public.
Position: This is a full-time position, with a typical work week being 40 hours total and overtime when necessary
Qualifications:
1. Current New York State LPN license required
2. Current Infection Control Certificate and Basic Life Support (BLS) certification is required
3. One (1) year of experience in medical/surgical nursing or in a long term setting
4. Must demonstrate an understanding of clinical risk
5. Must demonstrate consistent professional conduct and meticulous attention to detail
6. Must possess excellent verbal and written communication skills as well as excellent interpersonal skills with patients, staff and other health care professionals
7. Familiarity with EMR systems
Essential Functions:
Responsible for the following, but not limited to:
· Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned patient/family as evidenced by documentation and observation of positive patient care outcomes
· Performs and assists with clinical tests and procedures.
· Manages and operates equipment safely and correctly.
· Administers medications as per MD orders as per best practice, document administration , refusals etc.. , report adverse effects. Identify potential for medication errors and correct per policy.
· Observes, records, and reports changes in patients’ condition
· Performs treatments and special procedures using aseptic technique.
· Assists with personal care, such as helping with bathing and toileting. Demonstrates appropriate hand washing technique to ensure proper infection control techniques are followed throughout the assigned unit.
· Monitor basic patient health such as vital signs and overall condition. Document as indicated.
· Administer medication accurately per MD orders, observing patient response, and completing documentation in the medical record as indicated.
· Maintain and coordinate communication among patient/significant other, staff and physician
· Demonstrates proficiency in performing clinical procedures such as urinary catheterization, tracheal suctioning, sterile/clean dressing changes IV management and other clinical procedures as dictated by professional training.
· Enters documentation and data as required in the electronic medical record and related computer systems.
· Supervise certified nursing assistants and other non-licensed nursing professionals
· Take verbal orders from the MD and initiates orders as per policy
· Manages pharmacy such as calling pharmacies for orders , storing medications appropriately, remove expired medications, maintain the contents of medication carts and narcotic locked cabinets.
