Patient Care Coordinator
Quality Community Health Center, a FQHC operated for 41 years, provides service at 3 locations in Philadelphia, PA managing the health and wellness of patients from infants through senior citizens. We practice medicine as a partnership between us and our patients. Currently, we are seeking an Patient Care Coordinator to work within the North Philadelphia community. The ideal candidate will provide support to health center operations for established patients, have experience working within inner city area and possess a commitment to working to meet the unique needs of these communities.

POSITION PURPOSE: This position will work in conjunction with providers, patients, and support staff to coordinate and improve patient outcomes in medical and dental clinical care.
ESSENTIAL FUNCTIONS AND BASIC DUTIES:
1. Assess all Chronic Care, adherence, outcomes and initiatives in relation to patient care.
2. Assess medical and dental needs of all eligible patients in a timely manner.
3. Develop and coordinate comprehensive care plans for eligible patients.
4. Maintain consistent compliance with all QCHC operational policies.
5. Provide in-reach and outreach for all Chronic Care patients and tracking for outcomes.
6. Participate in patient case conferences with all QCHC Chronic Care related staff.
7. Serve as a liaison with agencies and related entities for patients.
8. Maintain and update accurately and timely database in relation to all Chronic Care statistics.
9. Maintain all standard trends and methodologies via educational workshops etc. for Chronic Care.
10. Provide and maintain professional conduct and customer service to patients and related professionals.
11. Prepare, monitor and submit reporting to address all needs for Chronic Care tracking.
12. Travel to all QCHC service areas as needed.
13. Adhere to all QCHC policies and procedures.
14. Other duties as assigned
