PRIMARY FUNCTION:

The Patient Care Coordinator in collaboration and continuous partnership with Entrpise Scheduling Team and SKCC to suyppoort access inititatives across the Enterprise.

S/he will coordinate scheduling patients between diagnosis and treatment, He/she participates in the assessment and ongoing support of patients in close collaboration with the the Care Team(s) s/he is supporting.

ESSENTIAL FUNCTIONS:

• Serves as the point of contact, advocate, and informational resource for patients with a non-oncology diagnosis,their care team, amp; family/caregiver(s).
• Promotes timely access to appropriate care such as helping to schedule appointments from first call to first treatment.
• Gathers records, organizes and supports care team.
• Monitors and promotes adherence to a care plan, developed in coordination with the patient, PCP, specialist and family/caregiver(s)
• Supports communication between the care team and the patient, facilitates screening and addresses barriers to care and documents all interactions in the patientrsquo;s electronic record
• Connects patients to relevant community resources, with goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs

• Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.

High school or GED required. Bachelor Degree preferred.

Two years experience in clinical or community resource settings; care coordination experience is desirable.
