POSITION SUMMARY

The Case Manager will serve as the youth and family's advocate for coordinating all necessary services, and will document the youth's referral to and satisfactory completion of each service related to specific court-ordered conditions. The Case Manager exercises discretion and independent judgment in the performance of the position while monitoring processes that ensures compliance with court-ordered conditions, community safety issues, and completion of all conditions leading to successful discharge for every youth entering the program.

Case Manager Duties:

• Perform assessment, supportive counseling, case management, referral, advocacy, monitoring and other related services for participants in the Low Level Community Supervision and Resource Connection Program.
• Provide in-home services to youths and their families, meeting weekly contact frequency requirements; develop treatment plans with youths and families based on individualized needs; make appropriate referrals to other service providers as appropriate and maintain weekly contact with provider to insure compliance; monitor school attendance and performance by weekly school visits with educators and guidance personnel.
• Interacts and coordinates case-related activities with the Department of Services for Children, Youth and their Families and the Mental Health, other government agencies, including law enforcement, and other social service providers.
• Attend regularly scheduled staff meetings, community events, trainings, and seminars.
• Is familiar with community resources and advocacy groups.
• Maintains a good driving record and upholds a good moral character during employment.
• Prepare and present cases at monthly Team Meetings.

EDUCATION & EXPERIENCE:

• Bachelors Degree in social work, psychology, criminal justice or a related field plus (1) year experience providing services to children/youth and families. Volunteer work can serve has experience 
• Experience working with children is required.
