Description

Case Manager:

This service is intended to increase skills to address the complex mental health and/or substance abuse needs of adults who have significant functional deficits in order to promote symptom reduction; assist recipients in acquiring mental health/substance abuse recovery skills necessary to successfully address vocational, housing, and educational needs; assist recipients in gaining access to and coordinating necessary services to promote clinical stability, and meet their mental health/substance abuse treatment, social, and other treatment support needs. The Case Manager assists in obtaining entitlements, resources, housing, educational opportunities, employment, etc. to enable the person to become as independent as possible

EDUCATION & EXPERIENCE:

• Bachelors Degree in social work, psychology, criminal justice or a related field plus (1) year experience providing services to children/youth and families. Volunteer work can serve has experience 
• Experience working with children is required.
