The Clinical Care Coordinator- Certified is the key liaison between the patient and their Patient-Centered Medical Home team. Team includes providers", clinic staff, and other health care professionals. Clinical Care Coordinator works to ensure safety, best practices, and high-quality standards are maintained for the patient across the health care continuum.

Required Qualifications:

• Completion of an accredited Medical Assistant program.
• National Medical Assistant Certification upon hire.
• National Provider BLS - American Heart Association upon hire.
• Obtain appropriate certification(s) within designated timeframe for a particular field of practice (for Specialty areas) upon hire.
• Audiometric," spirometry and EBT (evidential breath alcohol) testing to possess or gain the appropriate certifications (for Occ Health) within 1 year of hire.
• 3 years of recent health care experience working as a Medical Assistant.
• 6 months in specialty areas: recent experience working as a Medical Assistant.

Preferred Qualifications:

• College.
• Experience with electronic medical records and/or computerized billing systems.
