PROMESA R.H.C.F. is Hiring a LPN Near Bronx, NY

POSITION OVERVIEW:

Under supervision of the Program Director, the LPN is responsible for coordinating and delivering patient care utilizing the nursing process. The LPN participates as a member of the interdisciplinary team providing support and information about nursing services. The LPN coordinates care between other professional disciplines as needed. Through documentation and verbal communication, essential information is shared with health care team members. When care is not provided directly by the LPN assigns/delegates care to other qualified members of the health care team. This position includes no managerial or supervisory responsibilities.

KEY ESSENTIAL FUNCTIONS:

  • Follow accepted standards of nursing practice in providing direct care, performing procedures and 

using equipment. Ensure that the findings of the nursing assessment, the comprehensive care plan, and recommendations for nursing follow-up are documented dated, and signed in the client’s record

  • Assess clients at the time of intake, and then on an on-going basis if new medical or nursing needs are apparent, communicates to the client’s Primary Care Provider
  • Coordinates all necessary client services with the other interdisciplinary team members in the comprehensive care planning process
  • Provides nursing care to clients on a routine and crisis basis
  • Maintains accurate documentation of all nursing activities 
  • Administers STAT medication and renders treatment as prescribed by Primary Care Provider
  • Participates in the formulation and evaluation of effectiveness of nursing standards within the department and participates in appropriate Continuous Quality Improvement activities
  • With the Medical Director, maintains adequate infection control standards
  • Carries out established policies and procedures for medication and treatment administration
  • Make daily rounds to evaluate condition of all residents 
  • Prepares admission and discharge forms for residents to and from the hospital
  • Admits new residents and completes admission forms and nurses notes; orients residents to new environment
  • Documents pertinent information regarding resident conditions as per policy/procedure

REQUIREMENTS:

High School Diploma or GED in an equivalent related field required

3 years related experience

Nursing and substance abuse program experience

New York State Licensed Practical Nurse required

Excellent interpersonal skills and able to communicate both verbally and in written form

Commitment to cultural diversity and sensitivity
