Job Summary:
Case Management services help older persons with functional impairments gain access to appropriate services, benefits and entitlements needed to age safely at home and to maintain their quality of life. The Social Work Case Management Supervisor (Bilingual preferred), an MSW will have responsibility for supervising 5 case managers and will ensure that all case management clients receive a timely, thorough, strengths-based assessment; and that care plans are developed that reflect culturally sensitive options that make efficient use of community resources. She/he will be responsible for creating a training and development plan for each case manager on his/her team. Included in this plan will be assurance of completion of all NYC Aging training requirements and continued support of ongoing staff development.
The Social Work Case Management Supervisor will report to the Director of Community Services and will develop and maintain external partnerships with other organizations to expand resources and strengthen supports.
