- Managing personal queue in TruCare
- Outreaching to members to assist them with identified complex case management needs
- Documenting all interaction with members/providers/other team members in TruCare
- Calling providers to assist with appointments, prescriptions, other health related needs
- Assisting with SDOH needs including arranging transportation
- Checking and responding to email timely throughout the day
- Checking and responding to IM's timely during day
- Master's Degree in relevant field (Social Work, Psychology, or similar field)
- Experience in behavioral settings/with the service continuum.
- Experience & knowledge in community resources available Electronic medical record use/documentation Care/treatment plan development.
- Experience with completing comprehensive assessments Engaging challenging populations.
