JOB DESCRIPTION

TITLE:	Community Support Worker (PT)
REPORTS TO:	Clinical Therapist
APPOINTED BY:	CEO

BASIC FUNCTION: The community support worker (CS) works within a treatment team of the agency Kids Connected Program. Kids Connected is a high-fidelity wraparound program for children with highrisk intensive needs. The CS works with treatment team

in completing treatment plans and treatment plan updates within the Child and Family Team meetings.

1. Works with the therapist and child and family team to determine the quantitative goals of the child and family.

2. Provide medically necessary interventions in accordance with the treatment plan

3. Completes case note documentation following the standard format for the agency. Each case note needs to correspond directly to one of the treatment plan goals

4. Participate in monthly child and family team meetings to advocate for the child and family

5.Participate in weekly supervision sessions for h hour to 1 hour depending on the number of hours worked

6. Participate in training as indicated by the Kids Connected team

REQUIREMENTS:

Incumbent must have a bachelor's degree in psychology, social work, or counseling. Preference will be given to individuals with certification and/or training in strengths based, harm reduction, and trauma informed care. Individuals without a bachelor's degree but with significant experience in the child welfare or children's mental health field (more than 2 years) will be considered. Deep understanding and commitment to the issues facing homeless families.

Demonstrate sound organization skills including computer literacy

Demonstrate solid verbal and written communication skills
