**Job Summary:** We are seeking a dedicated Medical Social Worker to join our healthcare team. The ideal candidate will provide essential social work services to patients and their families, ensuring holistic care and support. **Duties:** - Conduct assessments to identify patients' psychosocial needs - Develop and implement care plans in collaboration with the healthcare team - Provide counseling and support to patients dealing with various medical conditions - Assist in discharge planning and coordination of post-hospital care - Maintain accurate medical documentation and records - Offer individual and group counseling for patients with diverse needs - Collaborate with medical professionals to address behavioral health issues - Provide addiction counseling and support for individuals seeking recovery **Requirements:** - Bachelor's or Master's degree in Social Work or related field - Valid state licensure as a Social Worker - Proficiency in medical documentation and knowledge of ICD-10 coding - Experience in psychotherapy, child and family counseling, and senior care - Strong background in providing individual and group counseling - Ability to work with individuals with autism spectrum disorder - Familiarity with behavioral health interventions and resources Join our team as a Medical Social Worker to make a meaningful impact on the lives of patients in need.
