Our medical practice is currently looking for an outstanding Certified Medical Worker to join our Care Coordinator team. The In Clinic Care Coordinator will work with the assigned in clinic and weight loss patients and is responsible for meeting the patients and providers daily needs to ensure these patients receive exceptional care. The ideal candidate will be responsible for:

• Appointment confirmations
• Same day urgent visit requests, triage and scheduling
• Complete orders given by provider (DME, Specialist referrals, letters, forms, obtain records, etc.)
• Add labs to flowsheet in EHR
• Prepare charts for next day schedule
• Answer incoming calls
• Enroll patients in CCM
• Ensure census is properly maintained
• Thorough, accurate documentation of all activities in EMR and CCIQ
• Ensure patients have a schedule follow up appointment
