Essential Duties and Responsibilities: The Coordinator of Shelter and Housing will be responsible for the overall direct service to Shelter and Housing clients; maintains records, supplies, equipment and maintenance for Tiny Home Community; and participates in Shelter and Housing program planning/implementation. This position does require working a minimum of one weekend day per week.
Responsibilities
Leadership
· Work in collaboration with leadership team to build strategic plans for mission delivery, meeting client and community needs and program growth.
· Establish a healthy, pro-active, team environment where mutual respect is expected and accountability exists.
· Responsible for developing staffing structure for optimal functionality and delivery of program services and achievement of desired program outcomes.
· Ensure services are supported by research.
Program Oversight
· Conducts case management meetings as required.
· Conducts staffing of cases.
· Assist in record reviews and internal audits. Assist with collection of charts documentation deficiencies.
· Participates in treatment team meetings with transitional living staff as needed.
· Completes interviews and screening assessments as needed.
· Attends all training and conferences offered to improve case management skills.
· Submits authorization requests for services within 14 day prior to expiration.
· Keeps staff informed of changes regarding authorizations and documentation.
· Communicates frequently with site/team regarding any updates and or programs changes.
· Maintains and supports relationships with clients and their supports.
· Provides initial information to all new clients assigned to caseload.
· Establishes and implements aftercare and follow up plans.
· Provides one on one orientation with residents, trains on daily livings skills, provides life skill events and trainings, identifies areas of need, and provides solutions.
Program Development
· Works with the TLP and Point teams in ensuring client satisfaction.
· Provides support through planning and coordination of services on behalf of the individual client.
· Provides advocacy for the individual client.
· Maintains confidentiality of the client and the client’s records.
· Writes, reviews, comprehends, and implements clients’ person-centered plans for all initial and updated plans. Monitors, educates, and assists those who are helping clients in developing skills.

· Drives clients to and/or accompanies them on scheduled and needed appointments.
· Provides monitoring and support in development of appropriate services for individual clients.
· Develops and completes discharge/transition plans for all clients under caseloads.
· Makes regular contact with client’s family members and/or work/school personnel involved with each client.
· Arranges person centered planning meetings, develops, coordinates, and writes person centered plans in a timely manner. Reviews plans and reauthorizes service within the appropriate timeframe.
· Attends staff meetings as scheduled.
· Maintains a professional positive relationship within the community as a representative of the agency.
· Performs other duties as required by the agency.
· Effectively follows policy and procedures abides by all guidelines, assignments, and directions set forth by the Director.

Education:Minimum of a bachelor’s degree in psychology, social work, or relevant field or work experience in Social Services
Certificates, Licenses and/or Registrations: Possession of valid Oklahoma driver's license and current vehicle insurance verification.
Skills and Abilities: Ability to plan, prioritize and organize work to maximize team performance and meet customer expectations. Excellent organizational skills and ability to work with attention to detail. Strong project management skills. Strong interpersonal skills and the ability to create and maintain collaborative work relationships within the organization and with external stakeholders. Refined critical thinking, analysis and problem-solving skills and the ability to establish and adjust to current and emerging priorities and negotiate multiple complex activities.
Training: Complete understanding of all agency programs and services, and any training needed as deemed appropriate by supervisor.
Other: Proficient with MS Office software, including Word, Excel, PowerPoint, and Outlook. Strong knowledge of federal, state, and local employment law
