POSITION DESCRIPTION

Responsible for interviewing, hiring, supervising and conducting performance evaluation for Certified Nursing Assistants. The Neighborhood/Household Nurse performs nursing functions with the neighborhood/household that includes passing medications, observing residents’ condition and communicating them to the Nurse Supervisor (RN), Director of Nursing or the physician as needed. The Nurse works closely with the Nurse Leader to ensure regulatory compliance, nurture a person-centered resident-directed culture, strengthens the ability of the neighborhood/household to grow as an effective home and promote individual and team learning. The Nurse is a teacher and role-model of professional nursing practices to the neighborhood/household staff and resident advocate for quality clinical care. The Nurse provides each resident with routine daily nursing care and other desired services in accordance with their assessment, choices and plan of care.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Ensures the clinical operations in the neighborhood/household are in compliance with federal, state, and local regulations and professional standards of nursing care while honoring person-centered and resident-directed care; observes resident status as necessary to assure quality care and coordinates with other resident services to assure continuity of care.
  2. Provides leadership to the neighborhood/household staff to maintain policies and procedures for clinical practices and neighborhood/household management that supports the values of PersonFirst and resident directed care. Assist with staff orientation and education as needed.
  3. Assures that documentation, medication, administration, resident nursing care, safety and sanitation, personnel and staff development and resident rights are in compliance with facility standards.
  4. Provide CNA assignments and evaluate the quality of care given by CNA’s.
  5. Check audit sheets daily and ensure CNA’s complete required charting.
  6. Responsible for conducting performance evaluations of Certified Nursing Assistants.
  7. Provide medications and treatments per RX by physician. Notify attending physician of any change in patient condition, incident, or weight change of five (5) pounds in one month. Make rounds with physicians, assisting when needed.
  8. Supervise the assigned dining room during meal periods.
  9. Revise and update patient nursing care plan and weekly summary as outlined for each shift. Transcribe doctors’ orders when appropriate. Fosters collaboration, consensus and negotiation to address problem solving in the neighborhood/household.
  10. Participate in resident conferences with team members as assigned.
  11. Attend in-service education programs and licensed staff meetings. Demonstrate carry-over of new or reinforce learning. Participate in establishing and improving standards of resident care by active contribution to creative ideas and sharing new innovations in nursing.
  12. Implement nursing policies/procedures such as bowel, bladder, skin care programs, restorative nursing, and orders drugs from pharmacy as needed.
  13. Complete incident reports with appropriate communication to M.D. P.R.N.
  14. Document on appropriate records of all nursing care given to each resident. Chart daily on Medicare residents depicting skilled nursing as ordered by physicians. Chart daily and as needed on all critical or terminally ill residents any change in patient condition for three (3) days after incident report, first three (3) days of admission. Record vital signs, output/intake, and administer oxygen if needed.
  15. Participate in Culture Change process through committee assignments that may involve Steering Committee, Action teams, PersonFirst team, and/or other self-led neighborhood/household team.
  16. Complete injury reports of employees and conduct investigation of employee injuries.
  17. Act as resource person for fire and disaster preparedness.
  18. Practice good body mechanics and use safety belt/support, gait belt as needed.
  19. Assist in answering call lights as needed.
  20. Attend meetings and serve on various employee committees as assigned. Participate as Safety Team Member.
  21. Proactively support the achievement of all team and organizational goals.

Qualifications


KNOWLEDGE, SKILLS & ABILITIES

  1. Possess a valid LVN license to practice nursing in the state of California.
  2. Possess current CPR certification.
  3. Three to five years prior experience in skilled health facility or retirement facility preferred.
  4. Possess leadership qualities, supervisory skills, interpersonal relationship skills.
  5. Working knowledge of Title XXII and OBRA regulations.
  6. Commitment to person-centered resident-directed care is essential.
  7. Ability to maintain confidentiality in a variety of circumstances and make sound judgments.
  8. Ability to report to work on time and complete the assigned work shift.
