POSITION DESCRIPTION 

The CNA provides medical and physical care to the residents of the neighborhood/household to meet their daily needs and enable them to function at the highest possible level. While this care is provided under the leadership of a licensed nurse, the CNA is the residents’ closest advocate in the neighborhood/household. The CNA works with the neighborhood/household team to meet the social service needs of the residents and of the neighborhood/household always providing support, friendship and kindness for each of the neighborhood/household members. The CNA plans and facilitates social activities for residents by providing meaningful ways to spend time as recreation, exercise, relationship building and fun. The CNA supports the neighborhood/household homemaker in maintaining a clean safe neighborhood/household environment.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Attend to all residents’ personal hygiene and personal care needs, including but not limited to grooming, hygiene, dining and nutritional, mobility, elimination, psychosocial and safety needs, and maintain daily charting as required.
  2. Organize all supplies necessary to complete the tasks. Support residents in self care, and provide care as necessary for those unable to care for their own needs. Follow universal precautions, proper infection control, sanitation and safety standards of practice in all work and activities.
  3. Observe residents closely, identifying changing needs and conditions at first indication, and report immediately to licensed nurse or neighborhood/household leadership as appropriate.
  4. Participate actively in the residents’ care planning and care conferences, serving as the residents’ closest advocate in directing their care and honoring their preferences.
  5. Consistently work with all household team members, led by resident and family preference to meet the needs and desires of all neighborhood/household residents.
  6. Attend shift report and receive assignments. Check in with residents when coming on shift and leaving shift observing call light placement, safety and comfort of residents. Respond to call lights of all residents regardless of assignment.
  7. Assist neighborhood/household team with meal set-up. Assist residents with personal care prior to meals and assist with meals along with providing fresh ice and water as needed.
  8. Provide and assist residents with passive and active range of motion.
  9. Observe residents for safety and positioning as needed encouraging appropriate movement to stimulate circulation.
  10. Assist in walking residents to and from designated areas for planned activities.
  11. Provide special care for residents as assigned by licensed nurse such as intake and output, and vital signs.
  12. Complete daily charting before leaving at the end of shift, reporting any changes to licensed nurse or Neighborhood/Household Coordinator.
  13. Document resident charges on appropriate forms when using supplies from supply room.
  14. Participate in the Culture Change process through committee assignments that may involve Steering Committee, Action Teams, PersonFirst Team, and/or self-led neighborhood/ household team.
  15. Attend scheduled in-service education programs to maintain certificate (24 in one year or 48 in two years), and demonstrate the understanding of information received. Participate in fire/disaster drills and be responsible for knowing policies and procedures.
  16. Use nursing policy and procedure manual for reference. Follows dress code.
  17. Attend meetings and serve on various employee committees as assigned.
  18. Responsible for practicing good safety habits and body mechanics. Wear safety belt/support when appropriate while in the Neighborhood/Household. Have gait belt at all times and use when appropriate. Participate as Safety Team member.
  19. Proactively support the achievement of all team and organizational goals.

Qualifications


  1. Possess a valid State of CA approved CNA certification card.
  2. Possess a genuine concern and interest for all residents in the Nursing Facility and have a strong understanding of and skills for caring for persons with dementia.
  3. Ability to write read and speak English.
  4. Ability to maintain confidentiality in a variety of circumstances and make sound judgments.
  5. Ability to report to work on time and complete the assigned work shift.
  6. Possess strong team skills in decision making and completing tasks in a team format.
