Job Overview:
We are seeking a dedicated and compassionate Hospice Social Worker to join our team of Hospice professionals. As a Social Worker, you will be a member of the interdisciplinary hospice team. The responsibilities of this position will be to assess the psychosocial affect of the terminal illness on patients, their families and caregivers. As the patient's condition progresses the MSW will help them to cope with disease progression and help the providing support and assistance families in need. Visits are conducted in patients' home, communities and facilities.

Responsibilities:

• Psychosocial assessment of patients and caregivers related to the patient's terminal illness and environment. Communication of these findings to the nurse and other members of the interdisciplinary team. The assessment will be completed in patient's residence. Social evaluations and planed interventions will be based on the findings in the evaluation. Counseling will be done for patients and family/caregivers as needed depending on difficulties.
• The social worker will provide services and information for practical necessities and will also serve as a liaison between patients, families, caregivers and other agencies.
• Contracts with public and private agencies will be maintained as resources for patients and other staff.
• Team members and physicians will be provided an understanding significant social and emotional aspects related to death/dying issues and other health problem's.
• Job-related tasks other than those stated in this description may be asked of the employee.

Job Type: Contract

Pay: $40.00 - $65.00 per hour
