Summary

The goal of the HOPWA (Housing Opportunities for Persons with AIDS) Transitional Housing program is to assist persons living with HIV/AIDS and their households in achieving and maintaining housing stability, thereby avoiding homelessness and improving access to, and engagement in, HIV/AIDS treatment and care.

  • The Community Social Worker delivers direct engagement and counseling services to clients in the HOPWA program. Provides outreach, motivational meet-greet, intakes, assessments, counseling, and warm-hand transfer to addiction recovery, and medical/clinical/behavioral health services within and outside the organization. Supports clients’ health, self-sufficiency, and housing goals. Helps improve personal empowerment and community wellness by encouraging safe behavior that decreases HIV transmission, and promotes the program’s partnership to maximize access to resources and client outcomes.

Essential Functions

  • High level of resourcefulness, patience, comfort, and humility in introducing self, services, and programs to clients in their environment, on an ongoing basis.
  • Develops and implements creative and trusting client service engagement opportunities for clients, and reaches out to individuals that are currently not receiving mental health services.
  • Greets, meets, and directs clients to appropriate resources available on-site.
  • Delivers motivational interventions for clients to facilitate acceptance of care for their needs, access to services, and resources, encouraging them to become integrated into services.
  • Provides support and guidance to clients with mental health disorders and/or trauma histories and/or addictions as they transition to recovery and/or behavioral health services, engaging those that are resistant to therapeutic services.
  • Facilitates or coordinates psycho-educational, informational, motivational, and community engagement activities geared to influencing access to services, opportunities, and resources.
  • Provides brief counseling to help clients transition to ongoing recovery and mental health services.
  • Serves as liaison between Case Management and the Health Center, and provides feedback and consultation to the Residential program. Support Case Managers in facilitating online clinic registrations, intakes, and releases to expedite access to clinical services.
  • Helps to maintain a positive, warm, collaborative environment alongside additional partners.
  • Participates as a member of a multidisciplinary team that prepares and presents client case reviews.
  • Documents services in the data system, maintains data, and reports outcomes.
  • Communicates regularly with the Supervisor concerning program and client issues.
  • On-time completion of assigned training and policies.
  • Performs other duties as assigned.

Qualifications

  • Associate degree in Social Work or related field, or certifications in a related field or areas of interest for the program (AOD, HIV/AIDS, Guidance, etc.) can be accepted
  • Four (4) years of experience working with underserved populations in a social service setting;
  • Clinical knowledge of substance use, mental health, trauma-informed care, and systemic understanding of homelessness
  • Open-minded and nonjudgemental working with clients with HIV/AIDS conditions
  • Intermediate user of MS Office
  • Ability to pass fingerprinting and background check upon employment
  • Must have a valid in-state driver’s license, provide current registration and insurance, and have a driving record with no more than 2 points within the last 3 years
  • Annual Tuberculosis screening and/or other screenings when necessary
  • Reasonable accommodations may be granted where appropriate
